Information Letter and Consent Form to take Photographs

Dear Site Coordinator and/or Supervising Teacher,
Part of this course is that pre-service teachers become familiar with the use of photographs as a form of documentation and assessment. While photos are an important part of learning we also want to make sure we are taking appropriate measures in assuring that such photos are used appropriately. Providing that you provide permission for the pre-service teacher to take and/or use photos for the purpose of documenting learning the following safeguards will be used. Photos of children/students taken or used will only be used for the purpose of assessment and making learning visible. Site and system protocols around the taking of photos will strictly be observed. Photos of children/students will not be taken or used where consent has not been granted. Photos will only be taken with the site’s camera or electronic device. Pre-service teachers are not permitted to use their own camera or electronic devices to take photos of children/students at any time. Digital images will be held by the preschool/school (DELETE whichever is not applicable) site. Pre-service teachers will only take possession of printed photos that have been approved by the preschool/school Site Coordinator or Supervising Teacher to be used for the purpose of documenting learning. Children’s/Students faces will not be shown in part or in whole in any photo. Photos will not be taken of children/students without their verbal consent on the day. Documented learning must be approved by Supervising Teacher or Site Coordinator. Photos of children/students will not be used beyond the parameters of this course. Our first line of protocol is to seek permission from the preschool/School (DELETE) site to take and/or use photos for the purpose of documenting learning to inform planning. If you consent for the preservice teacher to do so they will then send the attached letter to parents/guardians to also seek their approval. If the site does not consent to this practice the pre-service teacher will not send the attached letter to parents/guardians. While they will still be required to document learning they must do so without pictures of children/students. If parents/guardians wish to withdraw after saying yes, they may phone the Course Coordinator to let her know and the student will be directed to destroy any photographs taken that include the child. As a representative of your site could you please indicate if you approve or disapprove of the pre-service teacher to take and/or use photographs for the purposes identified, using the safeguards identified, please sign and return the section below so the University is aware that you have consented and are fully informed.
