Tracking Labs

To Download
• Download this spreadsheet at: www.kidney.org/atoz
• Read the instructions below to learn how to track important lab results and view trends over time
using this "Tracking Labs" spreadsheet.

General Instructions
• Click on the "Lab Results" tab (located at the bottom left of your screen). You will see an empty
sheet for recording lab results.
• After each health visit, record the date(s) and the test results. The dates must be typed in a 2-
digit format only. We suggest using month & year (for example: 1/12).
• Date must be typed in 2-digit format only. Otherwise the "Trends" tab will not align properly.

Looking at Trends
• To track changes in a specific test over a long period of time, click the "Trends" tab (located at the bottom
of this workbook). You will be able to see each individual lab result in graphs.
• To print your results, click "Control + P." A pop-up menu will appear. Under the "Print what"
option, select "Entire Workbook" or "Active Sheet."

Additional Instructions (For Patients)

• Do you know what your lab goals are? If not, print a copy of the empty lab results sheet and bring
it with you to your next healthcare visit.
• Ask your healthcare team about your goal for each item. Write them it in the "Goal" column of
this worksheet. Then type your goals into the "Lab Results" log sheet in the "Goal" column.

• Systolic blood pressure is the top number in a blood pressure reading. It represents the amount of
pressure blood puts on vessels when the heart is beating.
• Diastolic blood pressure is the bottom number in a blood pressure reading. It measures the pressure in
your blood vessels
