PETITION

COMES NOW, Petitioner through the undersigned counsel, in the

above-entitled case and to this Honorable Adjudication Board, most
respectfully states that:

1. Petitioner xxxxx is of legal age, Filipino, the spouse of the late xxxxx,
and resident of ___________, Iloilo;

2. Respondent Register of Deeds of the Province of Iloilo is impleaded

herein in its official capacity, being an indispensable party and in
order that there will be a complete determination of the herein case,
with address at ____________________ at which the respondent may
be served with summons and other court procedures;

3. That a Title has been issued and registered before the Registry of
Deed, Province of Iloilo in favour of _________, more particularly
described as follows, to wit:

TRANSFER CERTIFICATE OF TITLE NO. EPT No. _______

“Technical Description”
 Certified True Copy of the Title is hereto attached
as Annex “A” of this Petition.

4. It was found out in the EPT Title No. EPT-_______ that _________, of
legal age, Filipino, married to ____________ and a resident of Brgy.
________, Iloilo;

5. That said fact is not correct for said ________, of legal age, Filipino,
is actually married to _____________ and not to __________ as
evidenced by the Certificate of Marriage of __________ and ________
hereto attached as Annex “B” of this Petition;

6. That said ________ died sometime in August 2015. Copy of his Death
Certificate is hereto attached as Annex “C” of this Petition;

7. That the Certificate of Live Birth of ________ would show her correct
name. Copy of the Certificate of Live Birth is hereto attached as
Annex “D”;

8. That in the issuance of the said EPT Title No. EPT-_______, a mistake
was committed while typing the name of _______ by the Operations
Division of the Department of Agrarian Reform, Province of Iloilo;

WHEREFORE, premises considered, it is most respectfully prayed of

this Honorable Adjudication Board, that after hearing, Orders be issued:

1. DIRECTING the Registry of Deeds City of Iloilo to correct the EPT

No. ______ from __________, of legal age, Filipino, married to
__________ and a resident of Brgy. ___________, Iloilo to ________,
of legal age, Filipino, married to ____________ and a resident of
Brgy. __________, Iloilo which is the true and correct fact based on
the Certificate of Marriage;

2. Other reliefs just and equitable under the premises are also
prayed for.
