CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This contract executed and entered into by and between:

__________________________, an educational corporation organized and existing in

accordance with the laws of the Philippines, with principal office address at
________________________________, represented in this Agreement by its
__________________________, hereinafter referred to as the SCHOOL;

And

WHEREAS, the SCHOOL is in need of competent and qualified teacher/faculty to teach

and serve in its various Schools/Departments/Units/Offices;

WHEREAS, the FACULTY MEMBER represents that he/she is a competent and qualified
teacher / faculty and he/she agrees to be hired by the School.

NOW, THEREFORE, for in consideration of the mutual covenants and agreements made
herein, the parties hereby enter into a contract of services whereby the School hires the
FACULTY MEMBER/APPOINTEE under the following terms and conditions:

1. Academic Appointment:The FACULTY/APPOINTEE agrees to serve the SCHOOL

competently, conscientiously and faithfully as Faculty effective _______________ with a
rank of _____ which entitles him/her to the following compensation.

a. Basic Pay : Php

Provided that the services of the FACULTY MEMBER may be terminated even before the
lapse of the aforesaid period when a more qualified FACULTY MEMBER becomes
available or for any cause/s as may be provided by law in the usual exercise of
management prerogative;

2. The FACULTY/APPOINTEE agrees to teach the following subjects on the hour indicated
in the Faculty Loading hereto attached and made an integral part hereof:
3. The FACULTY MEMBER agrees to perform his/her duties and abide by the SCHOOL
policies, standards, rules and regulations as contained in the School Code or in any other
documents as may be promulgated by the SCHOOL authorities and the Board of Trustees of
the _________________________________ including but not necessarily limited to
preparation of the course syllabus, keeping attendance during regular class hours, giving
examinations, as scheduled and the correction of examination papers, submission of grades
within the prescribed period, supervision and/or viewing of films in the AVR, proctoring for the
quarterly exams, attending to various duties during enrollment and the like.

4. This contract of service shall be valid and binding for the entire period as stated hereon. In
case of resignation or voluntary separation from the service the faculty is mandatory required
to submit the formal written communication stating his/her intention to resign and the reason
for same at least 30 days PRIOR to the intended date of resignation or voluntary separation
from the service. Failure to comply with this mandatory requirement may subject the faculty to
damages and such other incidental expenses as maybe necessary and does not preclude the
school from taking legal action to enforce the matter.

5. The APPOINTEE shall undergo periodic assessment and evaluation of his/her

performance as a condition prior to renewal, as the case may be.

6. With the execution of this contract, the APPOINTEE hereby waives and makes a
quitclaim of whatever prior or existing rights, benefits, interest or claim arising of the previous
or existing contract that he/she may have or hold against the SCHOOL, its offices, employees
or agents and for this purpose, the APPOINTEE hereby represents and declares that he/she
has received wholly and completely by way of full and faithful satisfaction thereof such claim,
benefit, interest and / or other monies of whatever nature that he/she may hold the SCHOOL
responsible for.
