RE: Permission to Conduct Research Study

Dear Ms. Eula Fernando

I am writing to request permission to conduct a research study at your institution. I am currently teaching at
Pagalanggang National High School together with my students who are currently enrolled in the Agriculture Crop
Production and Science Technology Engineering Mathematics at Pagalanggang Dinalupihan Bataan in Belmont, CA,
and am in the process of writing my (i.e., Master’s Thesis). The study is entitled Soil Analysis.

We would also like to ask for your help to lend to us your materials for the said soil analysis. We know that your good
office has complete materials for the soil analysis and if it is too much for us to ask your assistance to use the different
materials that we will going to need for our research.

If approval is granted, student participants will complete the survey in a classroom or other quiet setting on the school
site. The survey process should take no longer than two weeks. Participants would complete the survey at school
premises. The survey results will be pooled for the thesis project and individual results of this study will remain
absolutely confidential and anonymous. Should this study be published, only pooled results will be documented. No
costs will be incurred by either your institution/center or the individual participants.

Your approval to conduct this study will be greatly appreciated. I will follow up with a telephone call next week and
