SUBJECT : BACK TO OFFICE REPORT OF NIA – REGION 3

Submitted herewith is the back to office report with the field / ocular inspection conducted in connection with
the implementation of ongoing projects conducted on December 3-7, 2018.

BACKGROUND

The field visit and validation was conducted to monitor the progress of works of the ongoing projects at NIA –
Region III in the provinces of Pampanga, Bataan, Bulacan, Nueva Ecija, and Tarlac respectively, which
contributes to the performance of the Agency.

As per Online Inventory of November 2018, Region III has an Operations monitored projects physical
accomplishment of 99.67%, 88.37%, and 60.54% for CY 2016, 2017 and 2018, respectively.

SMD – staffs arrived at NIA – Region III, Tambubong, San Rafael, Bulacan on December 3, 2018 and
proceeded to the Engineering and Operations Manager, Engr. Virgilio J. Ilao.

As per the Operations & Maintenance aspect of NIA – Region III, the following were the findings and
observations of this Office which were addressed to the Regional Office and IMOs.

ACTION TAKEN

As per exit conference, the Central Office personnel had emphasized to the Regional Office the commitment
to complete the carry over projects on or before December 2018. The Central Office personnel reminded the
Office to monitor / check previous submitted accomplishment report for counter checking of latest report.
Monthly accomplishment report submitted to the Regional Office by the IMO should tally with the actual
accomplishment of said projects and online inventory. The Regional Office was also reminded that the Online
Physical Accomplishment will prevail even the Office submits a hard copy report through the SMD – OD Office.
It was advised that all agreements, meetings between NIA, contractors, Irrigators Associations should all be
documented for perusal and references. It was also reminded by the Central Office personnel that Regional
Office monitor and check the encoding of physical accomplishment against the financial accomplishment
inputted by the IMO staffs.

REMARKS

Based on the status report as of November 30, 2018 for project implementation of Region 3 under GAA Local
Funds CY 2017 (see “Annex”), the overall physical accomplishment for Operations Department – SMD
monitored project was 88.37% comprising of different funding sources. Percentages (%) of areas for physical
accomplishment were as follows: 133.64% for new area in terms of hectares; 83.26% for restored area;
136.57% & 146.68% for canals and canal lining in terms of kilometers and 156.60% for structures in terms of
number per unit. The remaining 11.63% for physical accomplishment of project implementation were all
targeted to be completed by the end of 4 th quarter of CY 2018 based on the catch-up plan commitment by
Regional Office 3.

With regards to O & M activities, Region 3 Irrigation Management Offices (IMOs) have complied on a monthly
basis the submission of reports for the operation and maintenance of irrigation/drainage facilities of National
Irrigation Systems (NIS) as per MC No. 55 series of 2014.

RECOMMENDATIONS

It is recommended to strengthen the regular feedback / communication between the Central Office and
Regional Office / Field Office in the monitoring and evaluation of projects in order to be properly updated of
project issues and concerns and come up with immediate actions to address such.

1. It was observed during field visit / validation that remaining activities of work is 200 meters vertical
canal lining @ Station 6 + 400 – 6 + 600.
2. It was committed to be completed on or before December 31, 2018.

Findings / Observations:
1. For slope protection works (upstream San Rafael side), on-going concreting @ Sta. 1 + 300 – 1 +
500 of about 200 meters in length.
2. For remaining activities to wit: supply and installation of gabion boxes and mattresses; laying-in of
steel sheet pile and installation of railings.
3. The target date of completion of said project is on the 1st quarter of January 2019.

MARONQUILLO PIS, San Rafael, Bulacan

FINDINGS/OBSERVATIONS:
1. For engine and pump house facilities, construction works was already completed.
2. For canal lining, concreting of cut & cover canal @ Sta. 0 + 000 – 585 of about 585 meters in
length was also completed.
3. Remaining activities, laying-in of 36 inches diameter RCP of about 150 meters in length.
4. The said project was committed to be completed on or before 4th week of January 2019.

PAMPANGA DELTA RIS, SAN LUIS # 2, San Luis, Pampanga

FINDINGS/OBSERVATIONS:
1. For canal lining, construction of Class “B’ vertical canal lining @ Sta. 0 + 000 – 0 + 600 of about
600 meters in length was already completed.
2. Remaining activities, canal lining of about 100 meters in length.
3. It was observed that the canal lining has no backfill which may cause weakening on the sides of
the canal.
4. The said project was committed to be completed on or before January 2019.

PUGO PIS/CASONGSONG PIS, Cuyapo, Nueva Ecija

Project Cost: P 7,000,000.00

FINDINGS/OBSERVATIONS:
1. For the engine and pump house, on-going fabrication for steel frame of said pump house; for
canal lining, on-going construction @ Sta. 0 + 500 – 0 + 700 of about 200 meters length.
2. Remaining works, supply of engine and groundwater rotary pump.
3. The commitment of completion of said project is on or before 1st quarter of CY 2019.

SUMMARY/GENERAL RECOMMENDATIONS:
1. Fast-track implementation and completion of said projects to assure essential and sufficient
supply of water.
2. Close monitoring and supervision of the implementation of major activities as indicated in the
Program of Work (POW), plans, and lay-out by the In-Charge Project Engineer to assure the
quality and workmanship of accomplishments.
3. The RIO, IMO and the Contractor should be initiated a weekly regular meeting with regards to the
progress of said project in terms of physical and financial accomplishment.
4. The percentage slippages of project implementation for physical accomplishment were delayed
due to the schedule of water delivery for farming activities during wet season.
5. For additional item in the masterlist of activities, backfilling along both sides of canal lining should
be included for the protection of stability of canal structure.

CONG DADONG DAM

Arayat, Pampanga

As per attached memorandum dated September 3, 2018 form NIA Regional Office 3, RIM Josephine
B. Salazar, the final report for the conduct of geo-technical and structural assessment of PDRIS-Cong Dadong
Dam, the causes of crack at piers/gap of bridge girder due to heat of hydration. For the Alkali-Silica-Reaction
(ASR), there are no laboratory test and chemical analysis conducted to conclude the cause of cracks. For
shrinkage stresses due to heat hydration there are no cracks analysis.

In view of this, we also observed that the presence of gap of bridge girder (seismic joint) @ pier 4 as
shown in the photo. Therefore, we also recommended for the conduct of laboratory test and stress analysis.
