applicant for 19 years. Ms Grace Buan was one of the able and excellent teaching staff in a school I managed at
Concepcion West District, Tarlac Philippines.
2. Reason for leaving your employment: Resignation; Discharge; Lay-off/RIF; Military duty;

Resignation in lieu of termination/ discharge.


3. Duties and responsibilities during employment: other than an Intermediate teacher,Ms. Grace Buan was in-
charge of the Learning Area in Science and Health and a member of the Curriculum Development
Committee in the District.
4. How would you evaluate the general work performance of this individual? Ms. Grace Buan excels in
everything that she does. She can perform variety of duties and handles many situations with ease and is
comfortable with change. Under Ms. Buan’s guidance, achievement test scores of our learners improved by
25%.
5. What are the person’s strengths as an employee? She communicates well with her fellow staff and parents
and learners. She has great organizational skills and dedication as she gets the job done systematically and
go extra miles in achieving her assigned task/goals.

6. What is an area this person might want to improve upon as an employee? Her weakness is that she being a
detail oriented person as she sometimes focusses too much on the details on a certain task or program and
spend too much time analyzing specifics.
