DAY 1

OBJECTIVES:

Within the day, the students will be able to:

Observe courtesy call to LGU, MHU, to meet Barangay Officials, Health Workers, formal/non-
formal organizations; assess and survey different families/sitios; describe the characteristics of
the barangay and formulate initial plan for health and development of the community.

ACTIVITIES TO BE DONE:

 Courtesy call to the office of Mayor, Municipal Health Office and Barangay Council
 Review of secondary data/records and plans of each residence
 Transect walk
 House to house survey
 Spot mapping
 Documentation of ocular surveys

DAY 2

OBJECTIVES:

Within the day, the students will be able to:

Facilitate preparation of activities of the MHU; clearly figure out community health problem of
the barangay; collate gathered data accordingly; courtesy call and exploratory talk to
Elementary School officials and formulate initial plan of actions for health and development.

ACTIVITIES TO BE DONE:

 Facilitate/assist MHU activities


 Saturate the whole community through house to house visit
 Courtesy call/school visit and exploratory talk to the principal
 Finalized COPAR output from preliminaries up to chapter 2

POSSIBLE OUTPUT:

DAY 3

OBJECTIVES:

At the end of the day, students will be able to:

Accomplish chapter 1 and chapter 2 of COPAR output and community health problems will be
identified, presented and analysed with the core group to come up with community diagnosis.

ACTIVITIES TO BE DONE:

 Finalize the identified problems of the barangay


 Presented to the core group and collaborate with them in their field work activities
 To work out COPAR output requirements

POSSIBLE OUTPUT:

 To submit chapter 1 and chapter 2, COPAR output


 Finalized list of community diagnosis
FIELD WORK
MEETING WITH THE CORE GROUP

DAY 4

OBJECTIVES:
The students will be able to:
Facilitate Program of Activities at Barangay Oya-Oy Elementary School; Health Teaching and
Feeding Program, and strengthen Income Generation Project (IGP) through mushroom
productions with the core group’s full participation and cooperation.

ACTIVITIES:
 Coordinate and courtesy call to Barangay Oya-Oy and school officials
 Facilitate Program of Activities/Health Teaching on:
 Proper Hygiene
 Handwashing
 Toothbrushing
 Grooming
 The 3 food groups
 First Aid
 Bandaging
 Knot tying
 Poisoning
 Wound dressing
 Planting of mushroom collaboratively with the core group
OUTPUT:
 Educated and assisted 98 pupils for health and development
 Counselled and fed 15 underweight children of the school regarding the importance of
eating nutritious food
 Documentation of activities (pictures, programs, etc)
 Documented 8-10 plots planted with mushroom: A collaborative effort of BSN II and
Hawak-Kamay Oya-Oy Core Group.

DAY 5

OBJECTIVES:

At the end of the day, the students will be able to polish and analyse data gathering in
accordance to secondary data and actual survey with the BHW to finalize collation and
completion of data necessary in the research study, interpretation of data, initial lay out of action
plan addressing the identified community problems and formulation of necessary attachment
needed in the study.

ACTIVITIES DONE:

DAY 6

OBJECTIVES:

At the end of the day, the students will be able to:

Finish chapter 3, finalize action plan and accomplish appendices attached until chapter 3, and
continue strengthening of IGP Project.

ACTIVITIES TO BE DONE:

 Work out COPAR requirements through division of labor to finalize chapter 3


 Finish action plan addressing identified community health problems
 Accomplish necessary attachment for chapter 3
 Planting of mushroom collaboratively with the core group

OUTPUTS:

 Finished chapter 3
 Finalized action plan
 Accomplished appendices attachment until chapter 3

MUSHROOM PRODUCTION
OCTOBER 19, 2017

DAY 7

OBJECTIVES:

Within the day, the students will be able to:

coordinate with the principal for the monitoring and sustenance of health-related activities,
complete back logs of chapter 1,2, and 3 of COPAR output, initial layout of findings 1,
conclusion 1 and recommendation 1, restoration of herbarium and preparation of needed
materials for the continuation of mushroom production.

ACTIVITIES TO BE DONE:

 Coordinate with the principal for possible implementation of health work-related activities
 Completion of chapter 1,2, and 3 with action plan, preliminaries and appendices
 To restore herbarium at the barangay hall
 Continuation of preparation in the mushroom production
 Initial layout of findings 1, conclusion 1, and recommendation 1

OUTPUTS:

 Documentation of reports of planning and coordination with the principal outcome


 Completion of chapter 1,2, and 3 with action plan, preliminaries and appendices

Provide health care services to selected individuals, continue working collaboratively in the
production of mushroom, and finish findings/conclusion/recommendation 2 and 3 of COPAR
output.

ACTIVITIES TO BE DONE:

 Newborn visit
 Continuation of mushroom production with the core group
 Making of findings, conclusion, and recommendation 2 and 3 of COPAR output

OUTPUTS:

 Provided health care services to newborns


 Finished findings, conclusion, and recommendation 2 and 3 of COPAR output
 Documentation of newborn visit and mushroom production
