(“leased premises”) of the Registry of Deeds for Quezon City.

On ____________(date), you entered into a Contract of Lease with __________

involving the leased premises, which expired on 15 June 2015 or I have already bought
on ____________.

It appears that you have continuously refused to vacate the leased property
despite oral demand to vacate.

Therefore, a final NOTICE TO VACATE is hereby made upon you to vacate the
leased premises within a non-extendible period of fifteen (15) / thirty (30) days from
receipt of this letter. Your failure to heed this final notice will compel us to institute the
appropriate legal proceedings against you to protect our/my rights and interests as the
owner of the subject property.
