Requirements

a. Complete Standard Uniform (Ward Uniform)


b. Ballpens; Pencil, Small Ruler, Bandage
Scissors, calculator
c. Patient: Must be a non – BSN
student/graduate
d. Sterile and clean gloves (with extra)
e. Waste receptacles, Paper linings
f. Vital signs equipment
g. Handwashing materials
h. Cottons, pt. gowns, alcohols and etc…

SPECIFIC CRITERIA for GRADING
PRACTICAL EXAM

I. PROCEDURE (60%)
A. Safety (10%)
3%-Was the doctor’s order checked?
4%-Was aseptic technique maintained?
3%-Is equipment right?
B. Comfort (10% or 2% of each item)

2%-Was sufficient explanation done before the procedure?


2%-Was conversation used as a means of diversion during
procedure?
2%-Was patient made comfortable as possible for the
procedure?
2%-Was adequate privacy provided?
2%-Was waiting of patient minimized?
C. Economy (10 % or 2% of each item)

2%-Was everything in readiness before beginning


the procedure?
2%-Was there economy in materials used?
2%-Did the SN economize on time by observing
good body mechanics?
2%-Was average time used in the procedure?
2%-Was bed, bedding, bedside table, or other
equipment in patient’s unit protected or
unnecessarily soiled?
D. Good Workmanship (30% or 5% of each item)

5%-Was arrangement of equipment a picture of orderly


and a neat appearance?
5%-Did student show confidence and sureness in her
performance?
5%-Did he/she pay attention to details needed?
5%-Was skill and dexterity shown?

III. Written Examination (30 pts)

Intravenous Therapy
Administration of Blood and Blood Products
Respiratory Care
Pulse Oximetry
Oxygen Therapy
Suctioning the Naso-Oropharyngeal Airway
Providing Oral Care to Dependent Client
Urinary Catheterization
Administering Enema
Duration of Exam

30 minutes per student for actual


demonstration
1. Wear the COMPLETE STANDARD
UNIFORM with LIGHT MAKE - UP.
2. Come with patient at least ONE HOUR
before the scheduled practical exam.
3. If there is a need for you to take
medications (e.g., analgesics for
dysmenorrhea) please do so before the
practical exam. This is to avoid any
absences, tardiness or delays.
4. Students are grouped by four. The
schedule will be posted two days
before the exam.
5. Four (4) students will go up to the
Nursing Arts Laboratory & draw the
name of the clinical instructor whom
he/she will be reporting to; with
his/her client.
The 2nd batch of 4 students will ready
themselves at CN 101.
6. Upon knowing the Cl’ s name,
the student will
immediately report to the
designated area.
7. Read the situation and questions
carefully. The student may REVIEW the
procedure for 3 minutes which will be
counted as part of the 30- minute
performance. However, the
student who will take the
written exam is NOT allowed to do
reviews.
8. There will be no interruptions while the
student is performing the procedure
EXCEPT when the time is almost up or if
the procedure is not correctly done. The
student may or may not RECITE the steps
of the procedure while
performing it. You are graded according to
the ACCURACY and EFFICIENCY of your
performance. It is also expected that the
NURSING PROCESS is utilized during the
actual demonstration.
9. All the needed equipment and materials will
be prepared for by the CSR custodian. The
student will only have to ask the custodian
for the right tray to borrow. On the other
hand, the student should provide for his /her
own sterile/clean gloves, waste receptacles
and paper linings, mask, handwashing
materials, vital signs equipment, functional
calculator, etc.
The student must check the tray for the
appropriateness of the materials that will be
utilized.
10. Once your time is over or you are
through performing the procedure,
return the borrowed equipment to the
custodian or pass the test paper to the
CI. Once you are finished, exit SILENTLY.
Conversing with any student is STRICTLY
PROHIBITED. Once caught, sanctions will
be imposed (e.g. point deductions).
11. Cellular phones are NOT allowed during the
entire exam. Should there be a need to
compute, the student MUST BRING HIS/HER
OWN CALCULATOR. He/She will NOT
be allowed to leave the designated area to
borrow any equipment during the entire
period allocated. Extra sheets will be
provided as scratch.
12. Please attend to your own
personal needs prior to the
scheduled time. However, should
there be a need to answer the
call of nature; the Laboratory CR
is available for your use.
13. All students waiting for their turn
to perform must stay inside CN only.
Nobody is allowed to go up or loiter
along the halls.
14. It is expected that students
about to take an examination
MUST COME WITH A FULL
STOMACH. This holds true for
your respective clients also.
Miscellaneous

A. INFORM your patient way ahead of


time about your schedule for the
practical exam. This allows ample time
for him/her to prepare.
B. It is better if you have an ALTERNATIVE
CLIENT in case something unexpected
happens. Confirm with your CLIENT
first a day prior to the practical exam
if he/she will be available the following
day. Proper explanation on what to
expect during the practical exam
should be done to your respective
patients.
C. OBSERVE PROPER DECORUM. Laughing,
talking loudly, doing unnecessary things
and loitering are strictly NOT allowed.

D. Each one is expected to have STUDIED


and PRACTICED the procedure, its
underlying principles and related
literature (e.g., notes, handouts, books)
days before the practical exam.
E. PREPARE the necessary
equipment/materials (e. g., sterile and
clean gloves, waste receptacles, paper
linings, mask, handwashing materials,
calculators), and complete uniform the
night before the practical exam.
F. DO NOT forget to chart after you are done
performing the procedure.
Documentation is part of the 30 minute
demonstration of the student.

G. DO NOT exchange schedule with your


classmates. You must stick with the
assigned schedule. Failure to do so means
automatic zero for the practical exam.
H. Only VALID ABSENCES are accepted,
PLEASE NOTIFY US.
I. BSN students and graduates from any
nursing school are NOT allowed to
be used as clients.
J. DO read the criteria posted for
grading practical exam.
SAMPLE SITUATION

You are a student-nurse taking care of a


patient complaining of difficulty of
breathing. Upon assessment, you found
out that there are copious secretions in
the lung fields. What nursing interventions
should you perform?
