單位發現於2017年1-11月在使用居家化療攜帶
式輸注器病人，發生異常相關事件共10件，引發成
立專案改善。
問題確立
(一)病人方面：忘記衛教内容、不了解異常事件處置方式。
(二)護理人員方面：缺乏定期在職教育、非首療病人未再衛
教、因忙碌未雙人核對管夾開啟、敷料未固定好、因工作忙
碌僅採口頭重點式說明後請病人自行返家翻閱。
(三)衛教工具:衛教單張內容缺少返家管夾開啟注意事項、缺
乏清楚圖片解說及警示貼紙。
(四)政策方面：Infusor審核表內容缺乏管夾開啟評分項目、
缺乏敷料固定黏貼法照護標準。

2018年1月進行介入措施，3月統計使用居家化療 Infusor
為20人次，審核結果使用居家化療Infusor病人自我照護
之正確率從2017年81% 提升至2018年100%。
討論與結論
實施後，病人之居家化療infusor自我照護認知正確率
從81%提升至100%，進步率19％，達預期目標，2018
年1月審核20位居家化療Infusor病人自我照護認知正確率
達100%。
專案目的
1.設定「使用居家化療Infusor病人自我照護認知」
之指標為85%，故將目標值設定為100%；
2.異常事件發生率為0件。
改善方案：
(一)舉辦使用居家化療Infusor之在職教育；
(二)修訂居家化療Infusor病人衛教標準流程、簡易版衛教單QR
code網路連結及實體圖片、Infusor奶瓶警示貼紙衛教；
(三)修訂稽核居家化療Infusor病人自我照護認知正確率審核計畫；
(四)人工靜脈導管（Port-V）敷料黏貼法及長尾夾固定管夾使用方
式。
評值期：2018年2月1日至2018年2月28日
專案執行4個月後，統計2018年2月1日至2月28日居家化
療Infusor病人自我照護認知正確率，評值改善對策之成效，於
2018年4月將改善方案標準化，並落實於臨床工作，持續進行
監測病人自我照護能力。
針對居家化療Infusor病人於每次要接受治療到注射室預接
上居家化療Infusor時，由品管種子人員或護理師使用「居家化
療Infusor自我照護正確率審核單」進行評值；針對居家化療
Infusor自我照護認知查檢表衛教認知程度及異常事件發生率進
行調查。

2017年度發生人工血管（Port-A）相關異常問題共24 一.舉辦在職教育：於2018年1月22日上課。

一.病人方面：以乳癌居多，且為近期裝置之病人。 護理師。
