TETRALOGY OF FALLOT ( TOF) PATIENTS IN S ANGLAH HOS PITAL, BALI INDONES IA

Obje ctive
TOF is the most common type of Congenital Heart Defect (CHD) with overall occurrence is about 10% of
all CHD and 50% of cyanotic CHD. BTs is the most commonly performed systemic to pulmonary artery
shunt in TOF cases. BTs become an established palliative procedure with progressive outcome
improvements but it has disadvantages, such as the need for two operations, scars, volume loading of
the ventricles resulting high EF, and lower BP. This study aim to know the correlation between history of
BTS with Hb, Hct, EF and BP among TOF Patients in Sanglah Hospital, Bali.

A correlation study with a total sampling of 30 TOF cases registered in Sanglah Hospital's medical
records from 2015-2016 was conducted. Uncompleted medical records were excluded from this study.
Data analyzed using bivariate analisis.

Re s ult( s )
Patients age 9 m/o to 16y/o, equal in sex catagory, 36.7% born with low birth weight and 63.3%
underweight. Half of patients have high EF(>75%),63.3% have muscular type of ventricular septal defect
(VSD), 66.7% have severe right ventricular outflow tract obstruction (RVOTO), 30% with aorta-
overriding>50%, 20% have right aortic arch,70% have Major aorto-pulmonary collateral artery
(MAPCA), and 33.3% have high Hb and Hct level. Nine patients have underwent BTs, they're mostly a girl
(p=0.046; r=0.364) and born with severe RVOTO (p=0.01; r=0.463). BTs history have a significant
correlation with normal Hb and Hct level (p=0.012; r=0.463). There’re no significant correlation between
BTs history and low blood pressure (p=0.936; r=0.011), also high EF (p=0.625; r=0.030).

Co nclus io n
History of BTs has a significant correlation with normal Hb and Hct level as a sign that the patients have
improve their systemic oxygenation so they no longer need to compensate with increasing Hb and Hct
level. BTs has no significant correlation in decreasing systemic blood pressure and increasing EF in TOF
patients.
