1.0 Medical Fitness and Facilities

1.1 Contractor shall deploy only such personnel for performance of services who have been
certified to be medically fit by any MBBS doctor (provided that he/she use the same pre-employment
format as per Annexure V).

1.2 Contractor shall arrange for periodical medical examinations for all its personnel, which shall
be conducted at least on an annual basis or as demanded by OWNER in line with Gujarat
Factories Rules or other legal requirements. Contractor shall ensure that such medical
examination is endorsed by the occupational health clinic doctor nominated by OWNER

1.3 Contractor shall maintain adequate medical and first aid facilities at the Site, and off site
medical facilities, for its personnel, agents and representatives.

1.4 Prior to commencement of Services, Contractor will furnish to the OWNER’s representative
the name and contact numbers of persons who will be providing such medical services.
Contractor shall display the details of such persons prominently at the Site office, if any.

1.5 Contractor shall promptly inform the OWNER’s representative of all occupational illness &
injuries and shall furnish to OWNER a copy of its report in relation to any injury at Site
requiring the attention of a physician.

1.6 For personnel working in inert atmosphere, necessary medical examinations shall be carried out
by OWNER.

1.7 All contractor's workers shall report fit for duty. No one shall be allowed to work who is unfit
for work because of medical conditions, including the effects of medicines that impair the
worker's ability to perform his job safely. In case of contract workman reporting to duty after
two or more days of illness. Site In charge shall declare that the respective contractor worker
is fit for resumption of work. .

2. Site Restrictions

2.1 Unless specifically authorized, Contractor and its personnel shall not enter into or loiter in any
area of the site, except the Site where the Services are to be performed

2.2 Contractor shall ensure that Services are performed so as not to interfere in any way with the
traffic on any roads or footpaths at the Site, or in its vicinity or at any existing works, whether
or not the same are in the property of OWNER

2.3 Contractor shall ensure that its personnel, agents and representatives shall not bring to the
OWNER'S Premises.

i.) any firearm, ammunition or fireworks of whatsoever nature, or any other object which
in OWNER’S opinion may be a potential weapon; or

ii.) alcoholic beverages of any nature, lighters, matches, cigarettes, tobacco, non-
prescription drugs or any OWNER prohibited drug.
2.4 Contractor shall ensure that its personnel, agents and representatives shall not commit any of
the following activities at or around site:

i.) fighting, bullying, harassment, or other violent actions;

ii.) self-harm or attempted suicide;

iii.) vandalism or willful damage or destruction of property;

iv.) flagrant or deliberate violation of safety rules or procedures, in particular, any


unauthorized interference with critical safety systems; or

v.) negligence or willful misconduct in execution of Services.

2.5 Contractor shall ensure its personnel, agents and representatives to comply with the following:

i.) not to bring or use mobile phones within or inside the battery limits of the Site.

ii.) wear full IFR clothing (within ISBL) which is reasonably tight and other mandatory
personal protective gear when at Site in line with OWNER'S guidelines.

iii.) use mandatory personal protective equipment as per Site rules.

2.6 Contractor shall ensure that firefighting equipment is easily accessible at all times during the
work. In case of any restriction of access, it should be brought to the notice of OWNER.

3. Protective Gear and Equipment

3.1 Contractor shall ensure that all their personnel use safety equipment which is made available
for their safety of its personnel. Contractor shall maintain such equipment in a condition suitable
for immediate use. Contractor shall take adequate steps to ensure proper use of all such
equipment by its personnel.

3.2 Contractor shall provide all of the personnel deployed by it standard BIS / EN approved
personal protective equipment such as safety shoes, safety goggles, ear plugs, hand gloves,
safety helmet, rain gear, overalls, dust masks, and full body harness for use while performing
the Services as per job requirement. All contractor personnel shall use the mandatory PPE as
per the specifications of the PPE given in the Annexue-1. PPE used by the contractors must be
approved by the concerned OWNER officers. Penalty will be levied for use of substandard and
/or damaged PPE. Refer to Annexure-2 for the list of penalties.

3.3 Contractor shall obtain prior written approval in respect of each item forming part of the
personal protective equipment that is proposed to be provided by Contractor to their respective
personnel Contractor shall provide its personnel proper welding face shield attached to the
helmet for use during performance of any welding activity to prevent eye injury and burns.
Contractor shall ensure that all personnel engaged in grinding operations use face shields
attached to the helmet and use hearing protection (ear plugs) during performance of such
operations.
3.4 Contractor shall ensure that its personnel use safety appliances during performance of Services
as stipulated in the relevant Work Permit to ensure safe performance of Services.

3.5 Contractor shall ensure that special safety appliances provided by OWNER are used by its
personnel with utmost care. Contractor shall be liable and responsible for any and all damage
caused to such appliances.

3.6 Contractor shall ensure that their personnel engaged in mixing asphalt materials, cement and
lime mortars use protective footwear and protective gloves

3.7 Contactor shall ensure that their personnel engaged in washing and mixing or stacking of
cement bags or any materials which may cause injuries to eyes, inhalation hazard, use protective
goggles and dusk mask

3.8 Contractor shall ensure that their personnel deployed for stone breaking use protective goggles,
face shields and protective clothing.

3.9 Contractor shall ensure that their personnel engaged in spray painting or in dry rubbing or
scrapping of any surface having lead paint use suitable dust masks.

3.10 Contractor shall ensure that while Services are being performed in sewers and manholes, the
manhole covers are kept open and the manholes are ventilated at least an hour before the
personnel are required to enter the manholes (the atmosphere inside properly tested for
minimum oxygen content of 20% v/v), and the manholes so opened are cordoned off with
suitable hard railing and suitable warning signals or boards are provided to prevent accident to
the public. Contractor shall use the confined space entry permit to carry out such Services.

3.11 Contractor shall provide and keep ready for use all necessary safety equipment as may be
required when Services are being performed at a location where there is a risk of drowning.
Contractor shall take all necessary steps for promptly rescuing any person in danger.

4. Emergency Preparedness and Response

4.01 Notification of the occurrence of all incidents, such as property damage, injury or near miss
cases, either witnessed by contractor's workers or pertaining to contractor's activities, shall be
provided to the concerned OWNER Officials , contract sponsor or plant manager.

4.02 In case the incident involves contractor personnel, then contractor's manager or supervisor is
required to give his First Information Report (FIR) or preliminary investigation report to
OWNER within 24 hours of the occurrence of the incident.

4.03 OWNER reserves the right to conduct its own investigation. Contractor is bound to give all
information during the investigation of the incident to the investigating team. Contractor and
all his employees shall cooperate fully with OWNER in the investigation of any and all
incidents, no matter how minor.
4.04 Incase of accident, contractor should follow the ``HSEF Procedure for Incident reporting,
classification and Investigation"

4.05 Contractor shall carry out his own investigation and report findings to the concerned OWNER
engineer-in-charge within Five working days of the occurrence of the incident.

4.06 Communication, discussion, acknowledgement or any contact with the news media or any
publication company about any incident or any aspect of this contract shall be done by OWNER
only.

4.07 At all times, Contractor shall NOT disclose or publish any information relating to an incident
or any aspect of this contract to media sources or publication companies without written
approval of OWNER.

4.08 In the event of emergency, indicated verbally or by a fire alarm, all work permits issued to the
contractor and his employees shall stand cancelled immediately. The Contractor's personnel
must suspend all jobs, leave their job in safe condition and move to the nearest assembly point
in the plant and follow security instructions. Contractor supervisor shall prepare a precise
record of headcount of their personnel and inform the concerned OWNER Officials during the
emergency.

4.09 Hazardous work areas adjacent to or located in normal routes of travel shall be barricaded and
marked by reflective barriers as directed by the OWNER Engineer In-charge.

5. Permit System

5.1 Contractor shall perform or cause to be performed all Services at the Site in compliance with
the Work Permit System detailed herein below.

5.2 Contractor shall obtain relevant passes for bringing material and equipment into the Site and
obtain prior written approval from the OWNER Representative for removing any of its material
or equipment from the Site

5.3 Contractor shall obtain relevant work permits from the OWNER'S Representative. The permit
shall be issued in accordance with the work and safety norms and shall specify the validity
period and hazardous / non-hazardous area. Contractor shall display the permits for inspection
at all times when at Site.

5.4 Contractor shall comply strictly with all instructions set out in such permits, without any
exceptions. Contractor shall follow all requisite safety precautions as documented in the risk
assessment for the specific job they are assigned to carry out.

5.5 Contractor shall retain all permits issued in accordance with the Work Permit System for
inspection up to the permit closure.

5.7 Contractor shall ensure that its personnel participate in safety ‘tool box’ talks or meetings
intimated to it from time to time by OWNER. Contractor and its personnel shall at all times
comply with the warnings and instructions provided at the ‘tool box’ talks. Contractor shall
ensure that ‘Tool Box’ talk is delivered by the supervisor prior to commencement of Services.
Contractor Supervisors, shall also, ensure that activity related risk and controls are briefed to
the executors.

6. Contractor’s Tools, Tackles and Equipment

6.1 Contractor shall ensure that its equipment comply with statutory requirements as applicable.
Inspection certifications by competent agencies shall be available for inspection at all times.

6.2 Contractor shall use only such electrical portable tools as have been approved by the Owner's
Representative.

6.3 Contractor shall ensure that all equipment used by it are free from defects and properly
maintained and shall use only the appropriate tools for any job.

6.4 Contractor shall ensure that its personnel do not tamper or interfere with any machinery or
equipment at Site

6.5 Contractor shall ensure that:

i.) all electrical equipment used by its personnel are properly grounded and fitted with
proper plugs.

ii.) damaged or bare cables or conductors are not used by its personnel for any machine.

iii.) cables are laid preferably in one piece length to avoid points Joints and all cable joints
are properly insulated.

iv.) an ELCB less than or equal to 30 mA rating is provided.

v.) all portable electrical tools are inspected and sticker evidencing the same is pasted
thereon.

7. Barricading

7.1 Contractor shall respect and comply with all barricades placed to restrict access at Site.

7.2 Contractor shall place appropriate barricades as stipulated in work permits, risk assessments
and according to the procedures prescribed by OWNER to prevent other personnel from
entering into hazardous service zones.

7.3 Contractor shall, prior to commencement of Services, place appropriate barricades to eliminate
exposure of risk or harm to personnel taking into account impact of falling objects, pits,
excessive noise, sparks, welding slag, high pressure water jet, radiation, and so on.

7.4 Impact on other personnel because of falling objects, pits, excessive noise, sparks; welding
slag, high pressure water jet, radiation, etc. shall be taken into account before the start of the
work and barricades placed appropriately to eliminate exposure of other personnel to hazards.

8. Scaffolding / Work at Height

8.1 Contractor shall ensure that its personnel performing work at height should work on safe
platform/ scaffold erected in line with OWNER’s procedure in case if the job can’t be carried
out from the ground.

8.2 Contractor shall ensure that the scaffolding or staging is so fastened as to prevent it from
swaying from the building or structure. Competent persons shall properly design and certify all
suspended scaffolds.

8.3 Contractor shall not keep any loose items at height.

8.4 Contractor and its personnel shall use full body harness with fall arrestors and lifelines while
working at height to prevent fall from height. Record of inspection for full body harness with
fall arrestors and lifelines and test certificates shall be maintained and produced when
demanded.

8.5 Contractor shall provide its personnel temporary guard rails at the edges while working on
building floors to avoid falls.

8.6 Contractor shall provide proper safe approach such as temporary stairs or ramps to any
buildings under construction.

8.7 Contractor shall provide hand railings for all platforms and staircases soon after its construction.
Contractor shall prohibit movement on unguarded platforms and staircases. Contractor shall
plan all staircase erection in such a way that gratings and hand rails are provided along with the
erection of the staircase structure to avoid falls.

8.8 Contractor shall construct working platform, gangways and stairways in such a way that they
should not sag unduly or unequally and ensure that they are closely boarded, have adequate
width and are suitably fastened as described hereinabove.

8.9 Contractor shall maintain all scaffolds, ladders and other safety devised mentioned or described
herein in safe condition and Contractor shall not remove or alter any scaffold, ladder or
equipment while it is in use.

9. Electrical Equipment

9.1 Contractor shall ensure that the switch boards are provided with 30mA ELCB (RCCB) with IS
approvals. Contractor shall appoint certified electrical technician / engineer for any electrical
related jobs. Contractor shall ensure that all its electrical equipment’s are periodically inspected
and maintained in good working order and condition. Contractor shall only use IS approved
equipment’s and industrial fittings.

9.2 Contractor shall ensure that


i.) the motor, gearing, transmission, electric wiring and other dangerous parts of hoisting
appliances are provided with efficient safeguards,

ii.) hoisting appliance are provided with such means as will reduce to the minimum, the
risk of accidental descent of the load,

iii.) adequate precautions are taken to reduce to the minimum, the risk of any part of a
suspended load becoming accidentally displaced.

9.3 Contractor shall ensure that its personnel use necessary apparel (such as gloves and boots) when
such personnel are employed on electrical installation which are already energized. Contractor
shall ensure such personnel do not wear any rings, watches or carry keys or other materials
which are good conductors of electricity when they are at such electrical installations.

10. Enforcement of Safety Regulations.

10.1 OWNER may at any time and from time to time inspect and verify the safety measures adopted
by Contractor to ensure that all such measures are in compliance with safety procedures and
safety codes prescribed by OWNER, Applicable laws and Applicable Permits.

10.2 Non-compliance with the safety regulations set out in the Contract shall be deemed to be a
material breach of the Contract and OWNER may forthwith terminate the Contract. Such right
to terminate shall be in addition to any other rights or remedies available to it under the Contract
or in law

10.3 Contractor shall ensure that the supervisors appointed by it for supervising performance of
Services undergo the HSEF training (General Induction) organized by OWNER. Contactor
shall ensure job specific training to its personnel Contractors to communicate the entity’s HSE
requirements to their employees and demonstrate that they follow them. Contractors to confirm
at defined intervals that their employees are competent and their equipment is fit for service,
and their work is carried out in compliance with entity requirements.

10.4 Contractor and the supervisors appointed by it shall attend all meetings relating to safety and
environment conducted by the OWNER Representative or OWNER’s safety department.

10.5 Contractor shall submit to OWNER "MATERIAL SAFETY DATA SHEET" in respect of
chemicals, if any, used by it in performance of Services.

10.6 Contractor and its personnel shall comply strictly with all Site safety rules, cautionary signs and
verbal instructions issued by the OWNER'S Representative.

11. Housekeeping

11.1 Contractor acknowledges that good housekeeping is essential for safe execution of the Services.
11.2 Contractor shall ensure that its personnel maintain a clean and orderly work Site at all times
and properly dispose off any and all rubbish and trash.

11.3 Contractor shall ensure that rubbish, trash, debris, tools and/or equipment do not obstruct
walkways or stairways or cause tripping hazards at any time.

11.4 Contractor shall keep the Site and adjacent areas clean, tidy, free and clear of all waste, rubble
and hazardous substances and otherwise in a safe and workmanlike condition.

11.5 Contractor shall dispose of rubbish and other waste in containers provided at Site. On
completion of Services, Contractor shall clear away and remove from the Site all surplus
materials, wreckage, rubbish and other goods and materials belonging to Contractor and their
personnel and leave the Site clean, tidy, free and clear of all waste, rubble and hazardous
substances to the complete satisfaction of OWNER

12. Traffic

12. 1 Contractor acknowledges that only vehicles having valid access permit and drivers having safe
driving permit shall be permitted to enter the Site.

12.2 Contractor shall, at its cost and expense, arrange for transportation of its personnel within the
Site in closed vehicles (such as bus, minibus, jeep and car). Contractor shall not be permitted
to transport its personnel within the Site in dumpers, trucks, hydras, mobile cranes. Contractor
shall ensure that all transport vehicles arranged by it meet all applicable statutory requirements.

12.3 Contractor shall ensure that:

i.) vehicles being used by him at Site are in road worthy condition (especially their tires,
brakes, steering mechanism and lights) and have a reverse direction “audible alarm”.

ii.) the driver and front seat passengers wear seat belts.

iii.) wherever practicable, the rear passenger seats are also provided with seatbelts and if
such seat belts are provided, the rear seat passengers use such seat belts.

iv.) the spark arrester required for vehicles on ISBL-roads are approved by the Chief
Controller of Explosives and have CCOE approval markings. Contractor shall provide
the spark arrester for its vehicles.

v.) the speed limits indicated on the display boards on roadsides are followed by its
personnel.

12.4 Fines will be levied for violation of traffic and safety rules as specified by the individual OWNER
site.
13. Environment

13.1 Contractor shall develop a systematic environmental management plan in respect of the
Services to be performed under the Contract and shall at all times during performance of
Services comply with such plan.

13.2 Contractor shall in the performance of Services, take all necessary steps to prevent pollution,
maximise recycle, reduce and manage in accordance with applicable laws, wastes, discharge
and emissions.

13.3 Contractor shall conserve natural resources and if required for performance of Services use the
same in a responsible and efficient manner.

13.4 Contractor shall promote environmental awareness among its personnel and representatives to
ensure shared responsibility towards environment protection.

13.5 Contractor shall, and its personnel to, at all times during performance of Services under the
Contract, comply with the Environment Policy of OWNER as applicable from time to time.

13.6 Contractor shall comply with relevant laws and regulations as well as take any additional
measures, considered necessary.

13.7 Contractor shall follow a systematic approach to environmental management plan in order to
achieve continual performance improvement.

13.8 Contractor shall prevent pollution, maximize recycle, reduce wastes, discharge and emissions.

13.9 Contractor shall conserve natural resources by their responsible and efficient use in all our
operations.

13.10 Contractor shall promote awareness among his workers for shared responsibility towards
environment protection.

14. General

14.1 Contractor shall ensure that its personnel are aware of and comply strictly with the Site safety
precautions prescribed by OWNER including the special safety precautions prescribed in
respect of hazardous areas

14.2 Contractor shall ensure that its personnel are aware of the emergency assembly point.

14.3 Contractor shall maintain a register detailing the name and total number of personnel deployed
by it at Site.

14.4 Contractor shall provide skilled and trained personnel and ensure that its and their personnel
adhere to safe working procedures and practices to eliminate the possibility of any accident.
14.5 Contractor shall appoint a qualified safety supervisor as per the details mentioned in Annexure-
IV) for every 50 personnel deployed to ensure safety at the Site. Also, Contractor shall ensure
one job supervisor for every 10 workers.

14.6 Contractor shall ensure that adequate lighting is available at Site where Services are being
performed by it and

14.7 Contractor shall ensure that neither its personnel sleep or rest near the work areas such as below
scaffolding, near trenches, near roads, near equipment’s, near shade of vehicles or cranes.

15 Declaration of intent to bring materials to the site

15.1 Contractor must notify the OWNER’s EIC in writing of his intent to bring any material to
the OWNER site or premises to be used to accomplish his work or for any other reason.

15.2 An MSDS corresponding to each material intended to be brought to the site must be provided
to the OWNER’s EIC in advance.

15.3 The materials can be brought to the OWNER site after written approval is obtained from the
OWNER’s EIC.

15.4 All materials brought to the site, used at the site or stored at the site must be clearly labeled as
to composition. Waste materials must be properly labeled.

16. The Contractor shall abide by the LIFE PROTECTION RULES-ZERO TOLERANCE of
RIL.

These are the mandatory requirements. Breach of any of these rules will result in severe disciplinary
action, with the possibility of immediate termination of employment or contract, and dismissal from
the site.

Following are the 6 rules through which OWNER'S Premises intends to build Safety Culture of Zero
tolerance across the site.

1. No drugs or alcohol, No smoking, No use of mobile phones in hazardous area.


2. No Violation of Work Permit Conditions
3. No line break without authorization
4. No confined space entry without authorization.
5. While operating a vehicle on business, do not use mobile phones
6. When working at height, protect yourself from fall.

1. No drugs or alcohol, No smoking, Nonuse of mobile phones in hazardous area.

Below mentioned conditions and behaviors are prohibited and shall apply LPR ZT process on
violation inside OWNER'S premises.

" Using, possessing, selling or distributing illegal drugs.


" Remaining under influence of alcohol or drugs during duty hours. This includes remaining
under influence of medical drugs which may hamper the performance at work.
" Smoking inside Refinery premises
" Carrying or using mobile phone in restricted area. Use of mobile phone in safety critical jobs
may also result in distraction leading to an incident.
" Carrying any source of ignition such as lighters, cameras etc.

Refer the OWNER'S procedure HSE S 272 for getting approval on mobile usage and the areas
permitted / restricted for mobile usage at refinery complex

2. No Violation of Work Permit Conditions


" Never start the work without valid work permit authorization by area authority.
" Always comply work permit conditions and use Personal Protective Equipment (e.g. Online
BA/SCBA Sets, Safety Harness while working at height)
" Secure yourself and use Safety harness while working at height, as applicable.

3. No line break without authorization


" Never perform line break without valid authorization in hand.
" Always use recommended PPEs during line break as mentioned in line break permit (e.g.
Online BA/SCBA set, PVC suit, IFR, Alumina Suit etc.)

4. No Confined Space entry without authorization.


" Never enter inside Confined Space without proper authorization.
" Always ensure attendant remains present outside Confined Space and take precautions as
explained or written in Risk Assessment.

5 While operating a vehicle on OWNER business do not use mobile phones

Usage of mobile phones while driving causes major distraction and has resulted in many
incidents. While driving any vehicle, usage of mobile phone is strictly prohibited. Any
violation to this will fall under LPR-ZT. This is applicable for all the vehicles plying inside
OWNER'S premises.

6 When working at height, protect yourself from fall

Not anchoring full body harness while working at height has resulted in many incidents within
OWNER'S Premises. It is a cause of serious concern as the consequences in such cases are very
severe and often results in fatality of the individual. It is the responsibility of the individual
who is working at height to ensure that he/she is adequately protected and using full body
harness which is anchored properly at the right place so as to prevent any incident of fall from
height.

Classification of LPR - ZT Violations and Level of actions

It is every individual's responsibility to abide at all times by the above Life Protection Rules- ZT. Any
violation observed shall be classified with respect to potential consequences it could have caused as
well as type of violation. The consequence matrix mentioned below shall be used for classification of
disciplinary action.

Potential Consequences mentioned in the above matrix are self-explanatory and does not need further
explanation. Sabotage of all form shall be treated as Intolerable. Repeat violation by individuals will
lead to next higher level of disciplinary action.

Explanation on Type of Violations

" Unintentional, Situational, Exceptional:


Violation due to lack of clarity in communication or unintentional violation due to situational
difficulties or adversities.

" Routine:
Violation has become the normal way of doing things, everybody is doing it and it is usually known
by supervisors who ignore it by not acting on it to stop/prevent recurrence.

" Negligence/Shortcut:
Taking the shortcuts or narrowing the timelines in completing tasks at the cost of steps/ timelines
mentioned in procedures

" Sabotage:
Violation with intention to bypass procedure even after having awareness on procedural requirements

Annexure-III: Life Protection Rules – Zero Tolerance
Following are the six rules through which OWNER'S site intends to build Safety Culture of Zero
tolerance across the site.
1. No drugs or alcohol, No smoking, No use of mobile phones in hazardous
area.
2. No Violation of Work Permit Conditions
3. No line break without authorization
4. No Confined space entry without authorization.
5. While operating a vehicle on OWNER business do not use mobile phones

1. No drugs or alcohol, No smoking, No use of mobile phones in hazardous


area:

Using alcohol illegal drugs, or misusing legal drugs or other substances, reduce the ability
of the person to do the job safely. Smoking or use of matches or cigarette lighters could
ignite flammable materials. Therefore, nobody is allowed to smoke or use drugs or alcohol
and keep ignition sources such as lighters, matches, or mobile phones even in switched off
condition in their possession inside the ISBL plant, except in the area permitted.

Below mentioned conditions and behaviors are prohibited and shall apply LPR ZT process
on violation inside OWNER'S Complex
• Using, possessing, selling or distributing illegal drugs.
• Remaining under influence of alcohol or drugs during duty hours. This includes
remaining under influence of medical drugs which may hamper the performance at work.
• Smoking inside Refinery premises
• Carrying or using mobile phone in restricted area (other than intrinsically safe mobiles
authorized to key personnel). Use of mobile phone in safety critical jobs may also result in
distraction leading to an incident.

2. No Violation of Work Permit Conditions:

An approved work permit is required for all work to be performed except jobs which are
exempted from Work Permit Procedure. The permit to work recognizes the risks posed by
the relevant task to be performed and puts in place appropriate controls for the safe
conduct of the work right from the planning of the task.
Below mentioned conditions and behaviors are prohibited and shall apply LPR ZT process
on violation inside OWNER'S Complex
• Working without a valid permit
• Working without Lifesaving Personal Protective Equipment such as Online BA sets or
SCBA sets
• Permitting work without gas test, when required.
• Working at height without securing safety harness
• Permitting work without ensuring proper isolation (Process, Mechanical or Electrical).
• Walking on pipeline at pipe rack (1st tier or above) without using proper fall protection
• Performing Heavy Lift without Lifting Plan.

3. No line breaks without authorization:

Consider all process lines as full of hazardous material and energy until it is proved that
they are sufficiently isolated, depressurized, drained, purged for decontamination.
Below mentioned conditions and behaviors are prohibited and shall apply LPR ZT process
on violation
• Performing Line Break without valid authorization and risk assessment.
• Line break is performed without isolation, depressurization, draining and purging,
where line is likely to remain under pressure or contaminated due to passing or non-
provision of isolation valves.
• Lifesaving Personal Protective Equipment either not identified in permit or not used
during job (e.g. Online/SCBA Sets, PVC suits while acid handling, IFR /Alumina suits)

4. Entering a Confined Space without authorization:

Confined spaces in plants have potential hazards of Ignition, Oxygen deficiency, Toxic gas
exposure. Entering in to such confined spaces require proper planning for isolation,
depressurization, draining and purging them with suitable media. Confirmation of
atmospheric conditions through gas test is essential for ensuring safe conditions inside
Confined space before man entry. Below mentioned are the conditions/ behaviours which
are expected as a mandatory requirements and failure of which shall apply LPR ZT process
• Entering confined space without authorization
• Attendant of the confined space is not available when person is already working inside
the Confined Space.
• Process, Mechanical, Electrical, and Radiations Isolations not performed before
permitting Confined Space entry.
• Gas tests not performed for toxic, flammable and oxygen presence during permit first
entry and subsequently at predefined frequencies.

5 While operating a vehicle on OWNER business do not use mobile phones

Usage of mobile phones while driving causes major distraction and has resulted in many
incidents. While driving any vehicle, usage of mobile phone is strictly prohibited. Any violation to
this will fall under LPR-ZT. This is applicable for all the vehicles plying inside OWNER'S
Premises.

6 When working at height, protect yourself from fall

Not anchoring full body harness while working at height has resulted in many incidents within
OWNER'S Premises. It is a cause of serious concern as the consequences in such cases are very
severe and often results in fatality of the individual. It is the responsibility of the individual who
is working at height to ensure that he/she is adequately protected and using full body harness
which is anchored properly at the right place so as to prevent any incident of fall from height.
Annexure – IV

Contractor Safety Supervisor Competency & Validation Process

4.1.1 Safety Supervisor should have any of the following qualification &
experience:

• Qualification in Safety (Minimum 1 year fulltime course is mandatory) or


NEBOSH-IGC & above
And
• B.Tech:1 years of experience or
• Diploma (any engineering discipline) with minimum 4 years of industrial work
experience or
• Science Graduate with minimum 4 years of industrial work experience or
• Any other graduation with 10 years of industrial experience or
• ITI with minimum 10 year of industrial experience or
• 12th (any stream) with minimum 10 year of industrial experience

* Currently working Safety Supervisors are currently exempted from this


qualification criteria until they are not declared unfit for this role by OWNER'S
Steady State Safety department in terms of job role demonstration or any
violations etc.
Safety supervisors who possess the required qualification and experience have to
produce the documents which will be checked at PGRT by the Safety Training Officer.
Once verified, he will have to undergo screening test at PGRT (written & verbal) for
his placement as a Safety supervisor. Special camps may be conducted during
shutdown for screening as per the requirement.

4.1.2 - Evaluation of safety Awareness, Knowledge, and Skill & will of the Person
Validation based on a written test and a verbal interview shall be done by the
OWNER'S Steady State Safety Managers at PGRT once the person is cleared with
respect to the Qualification and experience criteria.

• Written Test validation/evaluation for new entry as well as working with steady
state. {Weightage 60%}
• Verbal interview based on job specific knowledge, safety attitude, interpersonal
skills etc. {Weightage 40%}
• Minimum 80% marks are required for passing.

The person who is taking the screening tests should understand that not all questions
will be applicable for every Safety Supervisor role, there would be some exceptions
based on the nature of work he would be assigned to. If the safety supervisor clears
the screening test, he is given clearance for his placement as a safety officer with the
respective contractor and area. If he is not able to clear, he is rejected and not
considered for the role for a minimum period of 3 months.
4.1.3 Competency Training
Safety Supervisor shall undergo Safety supervisor’s training/competence
development course at OWNER'S venue within 6 weeks of their joining.

Roles & Responsibilities of Contractor Safety Supervisor

1. Advise the Contractor in-charge regarding Statutory Requirements and Legal Requirements.
2. Advice and counsel team members on Safety Matters through meetings & Tool Box talks.
3. Provide Resource materials for Safety Training.
4. Attend training and get validated before start of the job as per the agreement during the bid
clarification meeting.
5. Impart training to contract manpower and validate them before start of the job under the
contract.
6. Lead Safety training program and conduct training at a fixed frequency
7. Ensure hygienic condition at work place.
8. Submit a report on corrective actions taken on safety observations against the contractor to the
on a quarterly basis.
9. Maintain Healthiness of all Personnel, PPE’s and Safety Equipment and their records.
10. Ensure all IFR suits available with contractor personnel are in good condition. Inform site-in-
charge for replacement in case any damage is observed in the suits.
11. Ensure that IFR suit provided is suitable for the job being executed by the contractor personnel.
12. Verify that all measuring instruments being used in field are calibrated and safe to use.
13. Assist the contractor supervisor in Risk Analysis of High Risk Jobs.
14. Provide training to contractor personnel to ensure competency of contract manpower
15. Attend training and get validated during major shutdowns / turnarounds before start of the
shutdown.
16. Participate in Plant Safety Assurance Team and Safety meetings.
17. Carry out a safety observation on a weekly basis in areas where the contractor operates.
18. Develop Safety audit plan and Participate in Safety audits.
19. Ensure implementation of Safety Audit findings and report to Site in-charge regarding pending
points.
20. Analyze Injury patterns and prepare resolution ideas.
21. Assist Site-in-charge in investigation of incidents.
22. Review all incident investigation reports and share the findings with contractor personnel
across various work locations in the site.
23. Participate in investigations of serious incidents.
