40 Hours at four (4) days – 10 hours per Day

SEMINAR FEE and COVERAGE

Eighty Five Thousand Pesos plus 12% VAT (PHP 85,000.00) for 25 participants. A charge of Three
thousand five hundred pesos (PHP 3,500.00) per head will be added in excess.
Fee is inclusive of seminar materials, resource speaker and Certificate of Completion.

 Your company shall provide the training room equipped with necessary training facilities such as
whiteboard, projection screen, and sound system (if training room is big) LCD, meals of Speaker
and Coordinator.
 Please provide us with the BIR Form 2303 or PEZA VAT exemption.
 Safety House, Inc will provide a Speaker (Safety Consultant / Safety Practitioner) and a
coordinator for the training.
 Topic sequence may vary depending on the availability of the Resource Speaker.
