PETITION

The undersigned petitioner, Deborah Micah O. Perez, unto this

Honorable Court respectfully states that:

1. Petitioner is a Filipino Citizen, single, over twenty one (21) years of

age, was born on October 17, 1995 , and a resident of #1475
Elizabeth Court, Suello Village, Baguio City, copies of the pictures of
the petitioner with signature at the bottom part are hereto attached
as ANNEX “A”, “A-1”, and “A-2”

2. Petitioner’s Telephone Number is 09088976816;

3. Petitioner has completed her law studies at Saint Louis University,

Bonifacio St., Baguio City Philippines;

4. Petitioner is a member of the Philippine Bar having been admitted to

the practice of law on May 6, 2019, as evidenced by a copy of the
Certificate of Membership attached hereto and made an integral part
hereof as ANNEX “B”.;

5. Petitioner maintains a regular place of work or business at 4th Floor,

CAP bldg., Post Office Loop, Baguio City.

6. Petitioner is a member of good standing of the Philippine Bar as

shown by the following certification:

a. Certification of no pending administrative case issued by

the Bar confidant’s office of the Supreme Court. (ANNEX
“C”);

b. Certification of no criminal case filed against her issued by

the Clerk of Court, Regional Trial Court, Baguio City.
(ANNEX “D”)

c. Certification of good moral character issued the executive

officers of the IBP-Baguio City Chapter. (ANNEX “E”);

7. Petitioner has paid her membership fee with the Integrated Bar of the
Philippines as per IBP Official Receipt No. 761273 dated January 4,
2019, a photo copy of which is attached hereto as ANNEX “F”. She
 has also paid the Professional Tax Receipt due as evidenced by the
PTR No. 144212 dated February 10, 2019, a photocopy of which is
attached herewith as ANNEX “G”.;

8. She possesses all the qualifications and none of the disqualifications

of a Notary Public;

9. She has not applied for commission before any other jurisdiction;

10. She undertakes to inform the Executive Judge within Ten (10) days
in case of any change in the given address and to submit her monthly
report religiously.;

11. She now desires to be granted Notarial Commission as Notary Public

for and in the City of Baguio.

PRAYER

WHEREFORE, In view of the foregoing it is respectfully prayed of this

Honorable Court after due notice, publication and hearing to grant this Petition
for Appointment of Notarial Commission of the herein Petitioner for and in the
City of Baguio.

Other just and equitable reliefs are likewise prayed for.

before the Supreme Court, Court of Appeals, or before any court, tribunal or
