AFFIDAVIT OF LOSS

THAT I, AGAPITO PARROCHO, of legal age, Filipino, married, and a resident of

Mainit, Looc, Sibulan, Negros Oriental, under oath, do hereby depose and manifest:

That I am a registered owner of a certain Toyota Jitney with Engine No. HW-
28468, Chassis No. KF10-105416, and Plate No. ACE676, registered under Certificate of
Registration CR No. __________ issued by the Land Transportation Office, Dumaguete
City District Office;

That sometime in the year 1995, while traveling to and fro Sibulan, Negros
Oriental and Bacolod City and upon arriving home in Looc, Sibulan, Negros Oriental, I
found out that the said Plate Number ACE 676 of my motorcycle Toyota Jitney was
already missing from its place;

That I tried my best to locate the said Plate Number but all my efforts were in
vain for I did not find the said plate number;

That the said plate number was not confiscated by any government authorities for
violation of any of our existing laws, but the same was lost;

That this affidavit is executed freely and voluntarily just to attest the true facts that
the aforementioned plate number ACE 676 got lost.
