THE NURSHING ASSESMENT OF THYPOID FEVER

Four areas that should be assesed :

1. History Of Desease

a) Past History

Assess the disease that has been experienced by the client, whether it has to do
with the gastrointestinal tract or not. Then review about medications commonly
consumed by the client, and also assess the history of allergies to the client,
whether allergic to drugs or food.

b) Current history

The patient is infected with salmonella thypi bacteria due to eating unhygienic
food. The patient said his body felt hot, nausea, pain in the abdomen. The patient
also looks weak and pale and the heat feels hot throughout the body

c) Family history
In family health, some have suffered from typhoid

2. Physical Assesment

a. General Condition
General condition What is the client's current condition, whether tired, weak or
seriously ill.

b. Vital sign

What is the client's temperature, pulse, breathing and blood pressure

c. Head

How clean the scalp, hair and head shape, whether there are abnormalities or
lesions on the head

d. Face

What is the shape of the face, pale skin / not

e. Eyes

What is the shape of the eye, anemic conjunctival state / not, jaundice sclera /
not, the state of the pupil, palpebra and whether there are disturbances in vision

f. Nose

The shape of the nose, the state of clean / not, there is / no secretions in the nose
and discharge, there is a sinus / not and whether there is interference in the
smell

g. Mouth

The shape of the mouth, dry / moist mucous membranes, dirty tongue / no,
whether there is redness / not on the tongue, whether there is interference with
swallowing, whether there is difficulty in speaking.

h. Neck

If there is swelling of the thyroid gland, is jugular venous distension found

i. Thoraks
What is the shape of the chest, symmetrical / not, examine the breathing pattern,
whether there is wheezing, whether there is interference with breathing.

j. Abdomen

What is the shape of the abdomen, dry skin turgor / no, whether there is
tenderness in the abdomen, whether the stomach feels bloated, do an
examination of bowel sounds, whether an increase in bowel noise / not.

k. Gentalia

What is the shape of the genitals, distribution of genital hair, genital hair color.
In men see the state of the penis, is there an abnormality / not. In women see the
state of the labia minora, usually the labia minora is covered by the labia
majora.

l. Instruments

Assess skin color, intact skin integrity / no, dry skin turgor / no, whether there is
tenderness in the skin, whether the skin feels hot.

m. Upper limb

Is there tremor or not, physical weakness, muscle pain and deformity.

3. Diagnostik Evaluation

X-rays are performed on the chest usually for part of the examination of all
significant febrile diseases.

4. Laboratory Study

Supporting examinations on clients with typhoid is a laboratory examination,


which consists of:

Laboratory Examination through:

a. WBC examination
b. SGOT and SGPT

c. Blood culture

d. Widal test.
