Dear Sir,

This refers to the above account in the name of my mother which we got opened in the year
2009. I stay in Dehradun and have been banking with BoB for over a decade.

My mother has been suffering from a terminal illness and in the process of shifting in and
out of hospitals , has lost the FDR of Rs. 4,00,000/= which was created in May 2016.

Her health has since declined rapidly. We contacted the branch for transferring of the funds
to her SA in the first week of August 2016 and also requested for a bank representative to
visit the home for signatures as she is not mobile anymore. Infact , she can only do a thumb
impression now, which was kindly agreed upon. However , the process could not move as
the said branch manager got transferred.

We now request you to immediately get the process underway as funds are required for her
treatment urgently.

 INDEMNITY LETTER In respect of lost or misplaced FDR

Wheras Bank of Baroda , Rishikesh Branch on May 2016 issued Fixed Deposit for Rs.
4,00,000/= only in favor of Mrs. Indra Kukreja through my SA # 25000100005753

And wheras the said Fixed Deposit Receipt has been lost or misplaced and wheras upon my
representation that the said Fixed Deposit Receipt has been lost or misplaced and has not
been mortgaged , pledged or assigned or dealt with in any manner and undertaking that if
the Fixed Deposit Receipt is found , it shall be returned to you for cancellation and at my
request , you have agreed to issue a duplicate fixed deposit receipt in lieu of the said lost or
misplaced Fixed Deposit Receipt.

Now , I , Indra Kukreja ( depositor) in consideration of the premises for ourselves and our
respective heirs , executors , and administrators jointly and severally agree and undertake
from time to time and at all times hereafter to indemnify and keep you indemnified from
and against all claims , demands , actions, liabilities , and expenses which may be made or
taken against or incurred by you by reason of the issue of such duplicate Fixed Deposit
Receipt or in respect of the afore said Fixed Deposit Receipt No.
