Five hours prior to admission, patient experienced persistent back pain, rated as 10/10,

radiating to the perineal area, not relieved by rest. There was also associated bloody mucoid
vaginal discharge, scanty, about 25Ml, non-foul smelling consuming 1 moderately soaked pad
with a whitish plug. There was perceived uterine contractions at an interval of 3-5 minutes
lasting for 1-3 minutes. Persistence of symptoms prompted the patient to seek consult in this
institution. Patient was then advised admission, hence admitted.

OB GYNE HISTORY:
MENSTRUAL HISTORY
Menarche was at 14 years old, lasted for 3 days, and used 4-5 moderately soaked pads, with
associated dysmenorrhea. Subsequent menses were regular, occurring every 28-30 days,
lasting for 3-4 days, and consuming 4-5 moderately soaked regular pads, with no associated
dysmenorrhea.

G1: Present pregnancy

GYNE HISTORY
No history of gynecologic illness were reported.

FAMILY HISTORY:
There is no family history of hypertension and diabetes mellitus reported.
