INVITATION TO COMPLETE HOUSING APPLICATION

Dear Applicant:

You previously signed up for the Indianapolis Housing Agency’s (IHA) Section 8 Project Based Rental Assistance waiting
list(s). The IHA now invites you to complete a formal application. You must attend and complete the necessary
paperwork in order to be determined if you are eligible to receive rental assistance at the site(s) listed below. Several
meetings will be held and are outlined below. You must attend one of these sessions to complete your application. The
meeting will last four hours. Be sure to attend a meeting that you know you can stay the entire time. If you leave early,
your paperwork will not be considered. If you are not able to attend any of the meetings, you must call
(317) 261-7210 by Sunday, December 1, 2019, to remain on the waiting list. You will not lose your position but will be
rescheduled for the next formal application meeting anticipated to occur in 2020.

The IHA provides rental assistance only. Parking for these meetings is your responsibility. The cheapest parking lot
(Capital Improvement Board Parking Lot 3) is on Missouri Street for $10 located between Maryland Street and South
Street. Walk on West Street toward Maryland Street to enter Exhibit Hall A. See attached map for clarification.

INVITATION TO COMPLETE HOUSING APPLICATION

To be qualified to receive rental assistance at the site in which you applied, you must:
(a) Bring all documentation listed below
(b) Meet the HUD requirements on citizenship or immigration status
(c) Not owe debt to any Public Housing Agency in the United States or its territories
(d) Not have any evictions from federally assisted housing programs since 2016
(e) Provide copies only of social security cards for all family members
(f) Meet the criminal history or alcohol abuse screening criteria (all family members 18 years or older)
(g) Not exceed the income limits for the rental assistance program

All member 18 years and older must be present for the meeting. Please review the attached notice and bring all items
that pertain to your household. The documents are necessary to complete your application and determine your
eligibility. Failure to appear during one of the available times may cause your removal from the waiting list.

DOCUMENTATION NEEDED FOR MEETING


IN ORDER FOR IHA TO DETERMINE ELIGIBILITY, YOU MUST PROVIDE COPIES OF ALL ITEMS
LISTED. FAILURE TO BRING COPIES OF ALL DOCUMENTS LISTED BELOW, WHICH APPLY TO
YOUR HOUSEHOLD, TO THE APPOINTMENT WILL DELAY THE ELIGBILITY PROCESS.

• Copies only of the Social Security Cards for all household members
• Copies showing proof of All INCOME and donations received by persons expected to live
in your household (example: employment (6 months of consecutive check stubs), TANF,
Social Security, SSI, Pension, Child Support (6 month statement), Regular Gifts, Income
from Temporary Services, Unemployment, Veteran Pension, Military Pay, etc.)
• Copies of Childcare Expenses
• Copies of any court ordered child support documents, if applicable
• Copies for 6 months of bank statements (APR-OCT) – just the first page is needed
• Copies of any other Assets (such as cash Certificates of Deposits, Money Market
accounts, cash value of life insurance policies, bonds, real estate value, etc.)
• Copies of your tax returns (“1040” form and “Schedule C” if applicable) for 2018
• Copies of Death Certificates, Marriage Licenses, Legal Separation documents, if
applicable
