I. Presence of Wellness Condition

POTENTIAL FOR ENHANCED CAPABILITY

Healthy Lifestyle

 Eats three times a day, eats nutritious foods.


 Usual foods are fish, vegetables, meat and rice.
 Drinks more than 6-8 glass of water daily.

Spiritual Well-Being

 She is a Roman Catholic.


 She goes to church every Sunday.

Health Maintenance/ Health Management

 Goes to the nearest Health Center when a member of the family is sick.

Availability and dissemination of Health Information

 Health Center of their Barangay is just walking distance from their home.
 Means of transformation is through public utility vehicle or solar jeepneys.
 Means of communication is through the use of their mobile phone.

II. Presence of Health Threats


A. Presence of risk factors of specific disease
 Stress due to lack of financial resources.
 Drinking of alcoholic beverages during the early months of her pregnancy.
C. Family size beyond what family resources can adequately provide
 The house consists of 1 room where the male members of the family sleeps there while
the women and children sleep in their living room/dining room which is made of woods
and covered with mat.
 The family has a lot of things which are not properly kept.
D. Accident Hazards
 Their house is made up of light materials which is a risk for fire hazards.
 They use “sungo” as their means of cooking their foods.
 Sharped objects were observed in going to their house.
 Their stuffs in their homes were piled up which is at risk for injuries.
F. Stress-provoking factors
 Care-giving burden in terms of taking care of their child especially when the child gets
tantrums because during the day, she’s the only one taking care of their child.
 Financial resources.
G. Poor home/ environmental condition/sanitation
 Inadequate living space
- They use their living room as multi-function area which may be a dining area and
sleeping area at night.
 Lack of food storage facilities
- The family uses “sungo” for cooking.
- There is no food storage facility such as refrigerator was observed.
 Poor lighting and ventilation
- Their house is full of things which are not properly organized that inhibits the light
and air from entering.
I. Unhealthy lifestyle and personal health practices
 Alcohol drinking

Cue: “nainom ako hin mga makahurubog nga irimnon han una, han bag-o pala ako
nagbuburod pero kun may okasyon la.” as verbalized by the patient.

 Inadequate exercise/ physical activity


- She doesn’t usually go out of the house when it is not needed or important. She only
stays at home watching the television and using with her mobile phone.
 Non-use of self-protective measures
- They don’t usually wear boots when walking around the flooded area/s.

Cue: “waray man kami botas, asya tsinelas la tam gingagamit. Waray gihap kami pumalit
kay talagsa man la kami bahaon dinhi haam.” As verbalized by the client.

III. Presence of Health Deficits


A. Illness states, regardless of whether it is diagnosed or undiagnosed by medical
practioner.

Cue: “waray paak kumadto sentro para magpa-prenatal, waray pa gihap ak magpalaboratory
pero gintagan na ako hin mga kailangan himuon, waray pa gihap kwarta asya waray la anay
pumakadto ha laboratory.” As verbalized by the client.

IV. Presence of Stress Points/ Foreseeable Crisis


 Marriage
- They are not married whether in civil or in church.
 Pregnancy
- The pregnancy was not planned.
 Additional member of the family
- Stress point because most of the time only the mother is the one taking care of their
first child.
 Financial Resources
- The main source of income comes for the client’s husband, her father-in-law and
from her brother-in-law. Their income is at minimum wage only which would not
suffice for the 8 members of the family especially that there are two children who
needs milk and also for the preparation of giving birth of the client. The total of their
income daily is Р935.00 daily.
SECOND-LEVEL ASSESSMENT
(Pregnant Woman)

A. RISK FOR ACCIDENT HAZARDS

E - Inability to provide a home environment conducive to health maintenance and personal


development due to inadequate family resources specifically financial constraints and limited
physical resources.

Subjective Cue: “burubarato man gud it sungo asya nga amo tam gingagamit tas nahadlok
gihap ak hit Shellane asya diri ak napalit hito.” As verbalized by the client.

Objective Cue: Daily income of working family members daily is Р935.00.

E - Inability to provide a home environment conducive to health maintenance and personal


development due to lack of knowledge of preventive measures.

Objective Cues: House is made up of light materials which is at risk for fire.

Doesn’t use boots when walking around flooded area/s.

Sharp objects were observed when going to their house.

Things at their house is piled up.

B. POOR LIGHTING AND VENTILATION

E – Inability to provide home environment conducive to health maintenance due to limited


financial resources.

Objective cue: house is full of things which are not properly organized that inhibits the light
and air from entering.

R – Failure to utilize community resources for health care due to lack of inadequate family
resources, specifically:

- Financial resources

Objective cue: Daily income of working family members is only Р935.00.

C. INADEQUATE LIVING SPACE

E - Inability to provide a home environment which is conducive for health maintenance and
personal development due to inadequate family resources specifically:

- Financial constraints
- Limited financial resources.

Objective Cue: The house consists of 1 room only.

Subjective Cue: “it mga lalaki adto ha kwarto nangangaturog tas kami nga mga babaye ngan it
kabataan adi la ha gawas, ito nga kahoy – mayda la ito mat.” As verbalized by the client.
