AFFIDAVIT OF NOTICE OF SALE

I, NOEL DALMACIO CRUZ, Filipino, of legal age, married and a resident of Sicayab,
Dipolog City, after having been sworn to an oath in accordance with law, do hereby declare:

1. That I am a duly authorized representative of the owner ROQUE MARTINEZ CRUZ by

virtue of a SPECIAL POWER OF ATTORNEY executed before Vice Consul Andrea
Lynn N. Lazaro, registered with the Consulate General of the Republic of the Philippines
in Los Angeles, California, USA with Doc. No. 8178, Page No. 82, Series of 2018.

2. That Roque Martinez Cruz owns of a parcel of land known as Lot 2558-L of the
Subdivision Plan (LRC) Psd 121433 being a portion of Lot 2558-L, Psd-75328, LRC Cad.
Rec. No. 756 situated in the Barrio of Sicayab, City of Dipolog, Island of Mindanao and
covered by Transfer Certificate of Title No. T-16071 issued by the Register of Deeds of
Dipolog City which consists of FIVE HUNDRED (500) SQUARE METERS;

3. That as Attorney-in-Fact, I sold the above-mentioned parcel of land to SPOUSES

AUGUSTINE E. GO and EMMA DY GO, and their children STEFFI SIOUX D. GO,
single and KATJA D. GO, single, all of legal ages, all are Filipino citizens, with residence
and postal address at 114 Bonifacio St., Dipolog City, by virtue of a Deed of Absolute
Sale of a Real Property executed on May 19, 2018 at Dipolog City before Atty. Nizza P.
Lesterio and recorded in her notarial book as Doc. No. 76; Page No. 77; Book No. 01;
Series of 2018;

4. That prior to the sale of said lot, the adjoining lot owners were notified of the sale of the
said lot by me or through my duly authorized representative;

5. That I am executing this Affidavit to attest to the truth of the foregoing facts and to
comply with the requirements of the Register of Deeds of Dipolog City.

IN WITNESS WHEREOF, we have hereunto affixed our signatures this ___ day of
