Please provide the following information to generate your software license. Review your choices carefully as a fee may be charged to reissue the
license. Use a separate form for each computer. If the software was purchased through a dealer you should contact the dealer for your license.

Email the completed form to license@csiberkeley.com. Your license will be sent to the email address you provide below.

Please choose type of license:

Standalone License (Standalone License is the only option available for CSiXRevit)
Network License Location of License Server (City, State)
The license will be locked to your computer’s hard disk (Disk ID) or to a Computer ID Key (USB Key). Refer to the Installation
Instruction Card to determine the Locking Code for your Disk ID or USB key.
Note: a Vista or Windows 7 license may require a USB key.

Provide the appropriate locking code (Disk ID locking is the only option available for CSiXRevit):
