2. Enclose the following in one sealed long brown envelope:

• Three (3) signed copies of your POEA Contract

• Two (2) signed copies of the Terms and Conditions
• One (1) signed copy of your updated Workers Information Sheet (WIS)
• Your original Seaman’s book
• Photocopy of your SSS or E1 form indicating your SSS number
• Photocopy of your PhilHealth ID
• Photocopy of your Pag-IBIG ID
• Photocopy / photocopies of bank book(s) OR validated deposit slip(s) indicating the bank account number(s)
of your allottee(s)

IMPORTANT REMINDERS: In consideration of the immigration rules of different countries, your Seaman’s Book must have a MINIMUM OF
SIX (6) BLANK PAGES.*To get your POEA e-Registration number and Worker’s Information sheet, log in to
https://eservices.poea.gov.ph/Home and complete your registration online. For technical concerns regarding your e-Registration number, you
