require complex care management

between all health care providers

been prevented wiuth better, collab, communication and coordination od care

Who is involved?
RN:
Role/scope of practice: the role of the rn is to provide health csre to individuals, families
and communities. Nurses provide services designated to promote health, prevent illness
and achieve optimal recovery from or adaptations to health probs
How profession collabs with others: nurses interact with other professions on a daily basiss,
nurses are present on a 24 hour basis and as a result often coordinate the health care team

Medicine (MD)
Roles/scope of practice: med is focused on the prevention, diagnosis, and treatment of
health isssues to imporves the wellbeing of people
How profession collabs: physicians have a large network of collabs in the delivery of health
care, depending on patients needs that span the spectrum from social work, counselling,
nutrition ect. Cannot provide complete, time appropriate, primary and tertiary healthcare
by themselves only

How is nursing involved?


Nurses are the largest group on health care teams with the most constant patient contact
