JAYBEE L. MAHUSAY, of Soong, Digos City, to be my true and lawful attorney-in-fact for me
and in my name, place and stead, to do and perform the following, to wit:

To make, sign , and claim/pick-up my Transcript of Record, Honorable Dismissal, and

other documents related thereto as I need it very badly being one of the requirements for me to
