Cases For Counseling

Case 1:

A 28 year old woman refuses to have sex with her husband who recently returned from
Singapore. She is eight weeks AOG and is afraid of contracting the Zika virus and having a
microcephalic baby. She will only agree if her husband uses the condom in the Zika prevention
kit.

Case 2:

A 32 year-old father of four has been diagnosed to have MDR TB but refuses to have treatment.
He lives in a single bedroom shanty with four kids, his wife, his two siblings and his parents. His
family is pleading with him to seek treatment but he refuses on grounds that it will interfere with
his work.

Case 3:

A 42 year old patient with TB has regular use of shabu. He has confessed during consultations
that he also supplies drugs to people in his community whenever he would be unemployed as his
work as a construction worker is seasonal.

Case 4:

A 33 year old schizophrenic mentioned that he will kill his ex-wife to gain custody of his
children. He is on medications and most of his statements appear lucid. It is the first time he has
mentioned such a thing and he has never had an instance of violent behavior in the past.

Case 5:

A 26 year old recently released from jail for molesting a child consult your clinic for cough. On
seeing his address, his apartment is situated beside an elementary public school. He currently
spends his time mostly at home as he is unemployed.

Case 6:

A 32 year old surgeon with HIV has had two instances where he has nicked his finger during
surgery and bled on the operative field. He has not disclosed this to the two patients nor the
hospital administration. He told you in confidence as you are his closest friend and he has said
that he has no plans of discussing this to anyone else in the future.

Case 7:
Your 80 year old mentor continues to perform surgery. There have been two instances wherein
he confusedly entered another surgical suite and two instances where his assist had two repair
nicked arteries that resulted in significant blood loss.

Case 8:

A physician colleague sells medications from his clinic that are unlabeled and only the
instructions for intake are provided. Of the branded medications, these are only available at his
clinic and cannot be found in the nearby areas. He has a monopoly of providing this service as
the nearest pharmacy is two towns or twenty kilometers away.

Case 9:

A surgical mission will go to a remote island in Batanes and perform cataract surgeries. The
team will stay for two days in the area. The only medical personnel available is the single
barangay health officer as there are no doctors or nurses in the community.

Case 10:

A surgical mission for thyroidectomies and hysterectomies will be sent to an area in the
Mountain Province. As there are no attendings available, only senior residents in training will be
sent.
