Bldg, Chand Nagar, Kausa, Mumbra, Dist – Thane. 400612, do hereby
solemnly affirm and declare as under–

2. That I have married to Mr. Noor ul Abul Hasan Sayyed on dated
18th of May 2017.
3. That previously my name was Shaikh Nausheen Shaikh Parvez
before marriage and after my marriage with Mr. Noor ul Abul
Hasan Sayyed my name has been changed as MRS. SAYYED
NAUSHEEN NOOR UL HASAN after marriage.
4. That now my new name is MRS. SAYYED NAUSHEEN NOOR
UL HASAN and will be used for all purpose in future in the
Government, Semi-Government, Non-Government and any other
Sectors.
