SWORN AFFIDAVIT

I, Mr. Waliullah Shafiullah Siddiqui, Age 48 years, Indian

Inhabitant, residing at – Sahara Apartment, Flat No. 404 on 4th
Floor, Behind Noor Baug Hall, Talao Pali Road, Kausa, Mumbra,
Dist – Thane. 400612, do hereby state on solemnly affirmation as
under:-

1. I say that, I have born on 30th day of June, 1971 at place
U.P, Taluka - Utraula
2. I say that, my full and complete name is Mr. Waliullah
Shafiullah Siddiqui, but my name erroneously written as
Waliullah Shafiullah in Secondary School Certificate and
Higher Secondary Certificate.
3. I say that, on the basis of the abovementioned Certificates I
have given the Apprenticeship Training Exam Under the
Apprentices Act, 1991, at – B.E.S.T Wadala, Mumbai –
400 031, of Mechanic Motor Vehicle for which I have gone
through the Training From 28/09/1990 to 27/09/1993 i.e
for 3 years, and got passed and awarded the Provisional
Certificate in the Year 1994, bearing the Exam Seat No.
MMV/60/Ex/161 under my Name i.e Siddiqui Waliullah
Son of Shafiullah
4. I say that, I am a Government Servant at present holding
the post of Sr. Mechanic Mv (P) in the M. V. Anik
Department, Sion, Mumbai. And my name is enrolled in
the Register and on other official records of office which is
as Mr. Waliullah Shafiullah Siddiqui since I am
appointed in the B.E.S.T Mumbai from the 1995.
5. I further say that, I am Indian Inhabitant, residing at –
Sahara Apartment, Flat No. 404 on 4th Floor, Behind Noor
Baug Hall, Talao Pali Road, Kausa, Mumbra, Dist – Thane.
400612, and having all the documents under my name i.e
Mr. Waliullah Shafiullah Siddiqui
a. Aadhar card – (Name - Waliullah Siddiqui / Aadhar

b. Pen Card – (Name – Walliullah S. Siddiqui / Pen Card

 6. I further say that, my name Mr. Waliullah Shafiullah
Siddiqui has been used for all purpose in the Government,
Semi-Government, Non-Government and any other
Sectors.
7. I further say that, I am making the Sworn Affidavit, for
getting Passport from the Passport Authority / Department
under my name i.e Mr. Waliullah Shafiullah Siddiqui.

Whatsoever stated hereinabove is true and correct to the best of my

knowledge, belief, and understanding. No part of it is false &
nothing material has been concealed there from. If found incorrect I
am liable for action.
