AFFIDAVIT

I, MR. SHAIKH AMIR SHAIKH SALEEM, Age 21 years,

residing at – Flat No. 203, Second Floor, Karinu Building, Rabodi,
K’villa, Thane (W), do hereby state on solemn affirmation as
under:-

1) I say that, I am permanently residing at the address
mentioned hereinabove i.e. Flat No. 203, Second Floor,
Karinu Building, Rabodi, K’villa, Thane (W), since my
birth, along with my family members.

2) I say that, I have School Leaving Certificate issued

by Municipal School of Thane Municipal Corporation,
which shows my name alongwith other details, copy of
which is enclosed herewith.

3) I say that, have no any documentary Proof of

Identification of myself and hence I am making this
affidavit to produce before the Concerned authority in
support of my I.D. Proof.

Whatever stated hereinabove is true and correct to the best of my

knowledge, belief and information.
