To reserve a spot in the league you must pay the full amount of $50. Registration is first come, first serve. Your spot
cannot be reserved until we have your $50 registration payment.
CASH PAYMENTS: Cash payments must be submitted at Anaheim City Hall, Community Services Department, 200 S.
Anaheim Blvd 4th Floor, Anaheim, 92805 along with this registration form. We do not accept personal checks.
CREDIT CARD PAYMENTS: After you have submitted your registration form, contact Arlene Newton at (714) 765-5290
or Matthew Morrow at (714) 765-3961 to make a credit card payment over the phone.

AMOUNT: $50 – Free Agent Registration Fee

ADDITIONAL INFORMATION: All free agent players will be assigned a team that will be scheduled a minimum of 10 games.
Each team is responsible for referee fees which will be $28 in exact cash each game to be paid at the field. All free agent players will
be given a reversible jersey that will be the same color as all other team members. Each jersey will have a number and size of your
choice (chosen above). All league schedules, standings, and stats can be found on our website at teamsideline.com/anaheim.
