Proceedings having been duly instituted pursuant to the provisions of the Alcoholic Beverage Control Law, (Chapter 478 of
the Laws of 1934, as amended) to cancel or revoke the above license issued to the licensee for premises located at the
‘address stated above and the licensee having duly plad "no contest” to the charges contained in the Notice of Pleading or a
hearing having been duly held by tie State Liquor Authority in connection with seid proceedings, and
The matter having been duly considered by the State Liquor Authority at a meeting hold on 12/18/2019, and tho

4. That on 3/4/2016 the licensee failed to conform with al representations set forth inthe application, or approved
amendments thereto, for the on-premises license under which euch license was applied for and issued, to wit: permitting
dancing, contrary tothe application; all cause for revocation, cancellation or suspension ofthe license in accordance with
Rule 64.8 of the Rules of the State Liquor Authority (9 NYCRR 48.8).
2, That on 3/4/2018, the licensee suffered or permitted the icensed premises to become disorderly in violation of subdivision
6 of section 106 of the Alcoholic Beverage Control Law.

4. That on or before 11/25/18, the licensee failed to conforrn with all representations set forth in the application, or approved
‘amendments thereto, for the on-premises license under which such license was appliod for and issued: pormitied dancing
‘and Dus, all cause for revocation, cancellation or suspension ofthe license in accordance with Rule 54.8 of the Rules of the
State Liquor Authority [8 NYCRR 48.8).
65, That on 11/17/16, in violation of subdivision 2 of section 106 of the Alcoholic Beverage Control Law, the licensee Kept

7. Thal on and before 03/28/19, the occurrence of noise, disturbance, misconduct or disorder inthe licensed premises, in
front of or adjacent tothe licensed premises, or in the parking lot of te licensed premises has resulted in the licensed
premises bocoming a focal point for police attention; all cause for revocation, cancellation or suspension ofthe license in
‘accordance with rule 38.1(q) of the Rus of the Stale Liquor Authority [9 NYCRR §3.1(Q)].
£8. That on and before 3/2819, in violation of subdivisions 1 and 3 of section 118 of the Alcoholic Beverage Control Law there.
has existed a sustained and continuing pattern of noise, disturbance, m'sconduct or disorder on or about the licensed
premises, related tothe operation of the premises or the conduct of its patrons, which adversoly affects the health, welfare,

40. That the licensee made its license availabe to a person not specified inthe license, in thatthe fcensee permitted Zhan

privilege from on and before 03/28/19, all cause for revocation, cancallation or suspension of the license in accordance with
‘section 111 of the Alcoholic Beverage Control Law.

lipulation of agreement, upon which any license or permit was issued or renewed by the Authority, or upon which any
