I request from Protos Ltd. to process on my behalf and for me the RE-EMBARKATION SERVICES , which contain, (but are not
limited to) the following services:
- confirming with Carnival Cruise Lines CM’s availability
- visa procedure assistance & filling up the visa application
- informing CM regarding documents’ validity requirements and procedures to be followed in order to be renewed
- sending to the CM the Re-embarkation procedure
- verifying re-embarkation documents validity as per CCL requirements
- sending the Seaman Book application to CCL office, if necessary
- processing and sending the confirmation sheet to CCL
- processing and sending the re-embarkation documents to the CM’s home address/or hand them over personal, if the
initial assignment is changed by Employer while CM is on vacation.

Deasemenea, subsemnatul, inteleg ca acest serviciu oferit este dorit de mine si acceptat la plata. Inteleg ca pot refuza acest serviciu si plata
lui, situatie in care voi purta intreaga raspundere, inclusiv financiara, asupra neindeplinirii intocmai a procedurilor de reimbarcare asa cum
sunt prezentate in scris (inclusiv electronic).
