An Examination of Personality Factors, Motivation and Outcome Associated with Dating Apps
Usage Among Senior High School Students in Cainta Catholic College

Dear Participant,

Greetings of Peace!

We are students of Grade 12 Our Lady of Miraculous Medal, from the academic strand Humanities and
Social Sciences. We are conducting a study about online dating applications and we are asking your kind
participation for our research study. You were selected as a possible participant because of year level
(Grade 11&12). Please read this form and ask any questions you may have before agreeing to participate
to the said study.

Background Information:

The purpose of this study is to examine the personality factors, motivation and outcome associated with
dating apps usage among Senior High School Students in Cainta Catholic College

Procedures:

If you agree to participate in this study, you will be asked to complete a given survey questionnaire and
answer as truthfully and completely as possible. This survey will take approximately 1-5 minutes. The
findings from this survey will be used to conclude the hypothesis of our quantitative research.

Risks and Benefits of Being in the Study:

This study has no risks.

This study has no direct benefits.

Confidentiality:

The information that will be given by the respondents shall be kept strictly confidential by the researchers
and will be used for research purposes only.

Voluntary Nature of the Study:

Your participation in this study is entirely voluntary. You may skip any questions you do not wish to
answer and may stop the survey at any time. Your decision whether or not to participate will not affect
your current or future relations with the researcher and Cainta Catholic College.

If you decide to participate, you are free to withdraw at any time without penalty. Should you decide to
withdraw, data collected about you will not be used.

You will be given a copy of this form to keep for your records.
