NARRATIVE ACCOMPLISHMENT REPORT

SCHOOL YEAR 2017-2018

I. Introduction
Libertad National High School, as one of the implementing arms of the Department
of Education in its threefold purpose in acquiring and disseminating knowledge, wish to
provide its students with a venue which promotes creative and innovative learning
environment.
In this connection, this accomplishment narrative report is done to report on the
progress and innovations made by the Teacher-Adviser in ensuring that quality
education is delivered.

II. Highlights of the accomplishments.

a. Classroom HRPTA Projects

1.As per mandated by Regional memorandum # 245 “Implementing


Guidelines on revitalizing classroom structuring cum Child-Friendly school system”;
the adviser, together with the students have maintained a well-structured
classroom and they managed to achieve 90 % of the required standard.
2. as seen by the parents, the comfort room unusable , so they have donated
an enough amount to repair the damage of the comfort room. Parents also donated
cash for the procurement of one orbit fan, repainting of comfort room, tiling of the
lavatory.

b. G8- Achievements

2.CPP best classroom implementer -3rd placer Grade 8
3. 92 % of the learners were qualified for honors for 4 quarters

c. Students records and monitoring of grades

1. On the student’s records, the teacher adviser have managed to enroll 100% of
the students in the online Learners Information System with minimal
information lacking in students profile.
2. Distribution of cards and follow ups.

Student’s card is a tool to keep track on student’s progress in his respective


academic subjects as well as its behavior inside the classroom as indicated in the
teachers comment and observation, thus, it is a must to regularly distribute cards
every quarter so that the parents will be informed about the status of their
children. Attached herewith, is a photograph of cards duly accomplished and
signed by the parents as proof of reception for your perusal.
3. Conduct of consultation and meetings.
The teacher-adviser also conducted several meetings for the report of HRPTA
contributions and students’ academic progress. Attached are the attendance
and minutes of meetings conducted. (See teacher’s portfolio)

d. Social responsibilities and Participation in various activities.


As a social well-being, the teachers and students should be indulged in different
social activities to develop confidence and camaraderie. As proof of participation,
attached are photographs of different activities participated by both teachers and
adviser.
