AFFIDAVIT OF DISCREPANCY

I, GLORIE ANN G. JANULGUE, of legal age, Filipino, and a

resident of Iligan City, having been duly sworn in accordance with law,
hereby depose and state:

1. That I am the said GLORIE ANN G. JANULGUE in the Birth

Certificate as herein attached in this Affidavit;

2. That in my Birth Certificate, my date of birth recorded was

“July 22, 1996” in the first page, but in the second page
where the Affidavit for Delayed Registration of Birth is found,
my date of birth recorded was “July 23, 1996”, as well as in
my Form 137, and Baptismal Certificate;

3. That I am declaring that my true and correct date of birth is

JULY 22, 1996;

4. That I am executing this affidavit for the purpose of my

application for VISA with Friendship Tours and Resorts
Corporation, to certify or attest to the truth of the foregoing
facts, and to explain the discrepancies in my date of birth as
reflected in the first and second pages of my birth certificate,
Form 137, and Baptismal Certificate.

IN WITNESS WHEREOF, I have hereunto set my hand this 6th
