2. Application completely filled is to be sent to Dte of Edu, AHQ Peshawar.

2. Following documents, in addition to those mentioned above, are required to be

submitted along with application forms:

(a) Attested photocopy of previous class marks sheet. The students of ‘O’ & ‘A’
levels (if applicable) must attach Equivalence Certificate issued by IBCC Islamabad.

(b) Wards of PAF retired personnel must attach photocopy of discharge / pension
book/pen-I of father / mother.

(c) Certificate of admission from the institution, indicating date of admission and
duration of course / class.

(d) Breakdown of fee structure duly signed by the competent authority concerned
(Headmaster / Headmistress / Principal / Head of Department / Registrar of the
College / University).

(e) Undertaking certificate duly signed by the student and countersigned by the
parents.

(f) Medical certificate by nearest service hospital in case applicant is

disabled/special child.
