Title of the Activity : SCHOOL MONITORING EVALUATION AND ADJUSTMENT FOR

THIRD QUARTER

Activity Objectives:

This activity aims to:

a. Conduct the Third Quarter School Monitoring Evaluation and Adjustment f

of this school year.
b. Accounting the targets and accomplishments of teachers for the third
quarter.
c. Checking and validating the targets and accomplishment.
d. Give technical assistance on some issues and gap of teachers on certain
KRAs
e. Finalize and submit the accomplished SMEA form

Design of the Activity: The conduct of the activity was done to evaluate and
validate the targets and accomplishments of teachers through MOVs of the
different KRAs.

Meeting Evaluation Rating:

End of SMEA Assessment – Rating is 4 – the conduct of SMEA for the third
quarter was a success the teachers were able to accomplished the form
thoroughly and was able to pass the SMEA form.

Narrative Report:

The teachers of Tatabunan Integrated School had a SMEA conference

together with the school head Mrs. Neresita S. Bechachino at around 9:00 in
the morning with the purpose of conducting the third quarter School Monitoring
Evaluation and Adjustment for the third quarter.

The school head presented and discussed the form to be accomplished

for the said SMEA conduct. She elaborated the strands of every KRAs for
teachers to be rated, evaluated, or adjusted. Every strand will be rated if 100%
accomplished, gain or gap. This will be supported of course with the
appropriate MOVs. Each teacher was given a copy of the SMEA forms to filled-
up and accomplished based on their evidences and MOVs presented. The
session ended at around 5:00 o’clock in the afternoon. Teachers were able to
accomplished and submit the SMEA form to our school head.
