TSSR Document Guidance and minimum standards of working safely and technically.

1 All Guidance pages including this page, Min. Tools Req'd page, Photos Guide pages, etc. to be deleted from the submitted reports.

All "Proposed information" particularly in the RF sections to be filled in prior to site visit for the Technical Site Survey (TSS), with such information to have been
2 provided by Vendor Radio Network Planning and Transmission Planning departments.
Vendor and/or Vendor Approved Service Provider (Subcontractor) must us qualified, competent, trained, certified personnel in the performance of the Technical
3 Site Survey, and production of the Technical Site Survey Report (TSSR).

Vendor and/or Vendor Approved Service Provider (Subcontractor) must work in accordance with International best practices for Occupational Health and Safety,
and all personnel going on sites must work in accordance with safe working practices, at a minimum in alignment with Indosat and Ooredoo group policy, manual,
4 procedures and safety instructions.
All personnel must be fully certified at a minimum to Indonesian regulatory standards and requirements, and furthermore, to Indosat, Vendor own and any
additional requirements, and have been provided OJT, and certified competent to perform the job both technically and safely by the Vendor.

Vendor and/or Vendor Approved Service Provider (Subcontractor) must provide at a minimum, the Indosat specified minimum tools, equipment, PPE and First
Aid kit required for TSSRs, and furthermore additional as recommended by the Vendor, and as required per the Site Specific Safe Working Method Statement
5 (SWMS), and Job-Safety Analysis, both of which also must be performed and approved by the Vendor prior to any personnel going on any sites to perform the
TSSRs.

Aside from the requirement of the main Vendor and their ASPs / subcontractors to have performed Job Safety Analysis and written proper Safe Working Method
Statements for each type of TSS to be done on each type of site, in generic form but specific to the area and situations and safety issues to be found in each area
6 and site type, enforcement of the usage of all required Personal Protection Equipment is mandatory, and the responsibility of the Vendor, including the wearing
and usage of personal radiation monitors, and the instruction of all personnel in the function and usage of these.

All photographs taken per the requirements of the various sections of the TSSR must be both included in the TSSR from a report perspective, in the specified
sections, and additionally, must be uploaded in the appropriate directory allocated to the TSSR within the Site Specific directory in Site Binders part of Share-
7 point. Guidance on the sub-directories required to be created and photos to be uploaded is contained within the Required Photos sections of this document, as
well as the specified sections for photo placement. Where in doubt, more photos should be taken, and uploaded.

All photographs must be taken with the minimum required capability camera, including the ability to geo-tag, time-stamp, and date-stamp the photos, with such
8 properties preserved in the raw photo upload to Share-point.

All photographs must be properly labelled, and for the raw photo upload, properly oriented for storage in Share-point, without loss or modification of Geo-
9 tagging, timestamp and datestamp properties.

Panoramic View (30°, 60°, 90°, 120°, 150°, 180°, 210°, 240°, 270°, 300°, 330°, 360°)
Panoramic views for tower/monopole site must be taken either from top of tower/monopole or above the existing antenna under survey. Panoramic views for
10 rooftop pole site must be taken from the center of building at highest point if possible. If it is not doable, panoramic views must be taken from each building
corner correspond to compass north reading. Sector views must be taken for existing antennas under survey in order to align the current coverage objective with
panoramic photos taken.

Earth resisitivity measurements must be made with the appropriate test equipment, and where there is no earthing in place as identified either visually or during
11 the earth resisitivity measurement test, Vendor must immediately notify Indosat Tower Management Group and Network Operations of this so that the site may
be rectified.

The TSSR must be saved in the proper directory for TSSRs in within the Site Specific directory in Site Binders part of Share-point, ie. Directory 22 TSSRs (all SoW).
13 It is within this directory that a sub-directory referring to the specific TSSR, e.g. TSSR_RMBS_2013 or TSSR_RANM_2014 shall be created, and under that, the raw
photos uploaded into the specified sub-subdirectories.
14 For Transmission specific TSSRs, two separate site reports may be combined for submission of a single "link" or "hop" TSSR.

Pada hari Senin sampai dengan hari Rabu atau tanggal 24 September 2018 sampai dengan 26 September 2
telah dilakukan Insatalasi Perangkat Radio Broadband Wireless Access (BWA 10.5 GHz). Adapun data dan
kelengkapan perangkat beserta konfigurasi site Base Station terdokumentasi dalam laporan “Dokumen Insta
Site” yang telah disusun berdasarkan data lapangan dan juga telah dilakukan pengecekan secara bersama
yang ditunjuk sebagai Waspang yang melakukan supervisi terhadap pelaksanaan kegiatan Instalasi Site Bas
Station ini

Adapun segala yang terkait dengan informasi Site Base Station terkait, secara lengkap tertuang dalam dokum
ini, dan apabila ada penambahan dan perubahan terhadap perangkat ataupun konfigurasi Base Station terka
