CONTRACT OF EMPLOYMENT

MADE AND ENTERED INTO BY AND BETWEEN:

BEAUTIHOLICS WELLNESS & SPA


with address at:
2nd floor GMC Bldg. Cagayan Valley Rd. Poblacion San Ildefonso Bulacan herein represented
by Ms. Rachelle Anne Magbitang, duly authorized hereto (hereinafter referred to as the
“EMPLOYER”)

AND

-----------------------------------------------------------
(hereinafter referred to as the “EMPLOYEE”)

WHEREBY THE PARTIES AGREE AS FOLLOWS:

1. APPOINTMENT
The EMPLOYEE, who hereby accepts the appointment and is appointed as a SPA
ATTENDANT for the EMPLOYER.

2. DURATION

2.1 This agreement will become affective as from _____________________________ and it


will be valid for 6 (six) months.
THE EMPLOYEE’S DUTIES

3.1 The core of the EMPLOYEE’s duties towards the EMPLOYER is a duty to obey all lawful
and reasonable order and to perform such work as she / he is directed to perform which
falls within his / her vocational ability.
3.2 EMPLOYEE is tasked to:
Be punctual and be at the spa 30 minutes to 1 hour before the opening time (provides
time for cleaning).
Maintain the spa as neat as possible.
Perform tasks of an all-around spa attendant (nails, facial, eyelash, etc. )
Be responsible in addressing personal issues with co-workers or managers.
Maintain a quiet and peaceful atmosphere whenever there are clients.
Know all services and are able to explain in the best of the EMPLOYEE’s abilities.
Follow the decking cycle.
Abide to old and new spa regulations.

3.3 Without limiting the aforesaid duties, the EMPLOYEE is obliged to strictly comply with the
provision of this agreement, may not misappropriate the EMPLOYER’s property, keep all
information entrusted to him / her confidential and have to adhere to the general Code of
Conduct that governs all relations with co-employees, clients and patients.

4. SERVICE HOURS

5.1 Service hours will be 6 or 7 days a week. Hours vary depending on spa’s operating hours.

6. REMUNERATION

The EMPLOYEE will be entitled to the following remuneration:

6.1 A daily base rate of Php _______________. 10% Commission on services (Please see
section 9 for exemptions) Review will take place every end of contract and any appraisal will
be based on employee performance as well as the spa’s overall sales performance, the
average will have to be a gross sales of Php 150,000/month to consider appraisals.

6.2 A thirteen month pay equal to one month’s salary payable at the end of every year during
December. This bonus will only be paid after the completion of a twelve (12) months
continuous service with the EMPLOYER.

6.5 SSS, and Philhealth benefits will start as soon as possible. There will be an employee-
employer portions for these benefits.

6.6 The EMPLOYEE hereby gives permission to the EMPLOYER to deduct all obligatory
deduction as authorized by statute from the above clause.

6.7 Cash advance: A cash advance limited to Php 1500 is allowed per employee and this
cash advance is not to be renewed until fully paid. Any amount larger than Php 1500 will
be a LOAN instead and is applicable to 3% interest a month. The maximum loan amount
will be Php 6000.
7. ANNUAL LEAVE & SICK LEAVE (AFTER 1 FULL YEAR OF EMPLOYMENT)

7.1 The EMPLOYEE is entitled to five (5) days SICK or VACATION leave on full pay for each
and every annual leave cycle.

7.2 The said vacation leave shall be granted by the EMPLOYER as from a date determined
by the EMPLOYEE at any time during the 12 months cycle.
7.3 This said Leave is not to be in conflict with other employee’s filed leave.

8. PUBLIC HOLIDAYS

The Spa will CLOSE earlier (6 pm cut off) during public holidays but may open earlier
depending on the announced time of operations.

9. QUOTA & COMMISSIONS


9.1 There will be a Php 1000 sales quota per person per day to be able to avail their 10%
commissions.

10 TERMINATION

10.1 This agreement may be terminated by either party by giving a one month’s written notice
of termination of service the one to the other, provided that such notice must be given on

the 1 day of the particular month.

10.2 The period of notice shall not be given during the EMPLOYEE’s absence on leave as
determined herein.

11 CERTIFICATE OF SERVICE

On termination of employment an EMPLOYEE is entitled to a Certificate of Employment.
