A GUIDE TO THE PUNLA/PLEA ENROLLMENT FORM
IMPORTANT NOTES TO REMEMBER
• Middle Name. The borrower must fill-in their complete middle name, not their middle inital.
• Contact Number. If the borrower does not own a mobile device, they may provide the contact number of any member of their household.
• Household Head. The head of the household is an adult person, male or female, who is responsible for the organization and care of the
household, or who is regarded as such by the members of the household.
• Barangay Verification. If the borrower is able to present either a valid Voter's ID and/or a Barangay certification/clearance as proof of his
identity and residence, a photocopy of such documents (front and back for ID cards) shall be attached to the enrollment form and thus,
verification of Part I by the Barangay is no longer necessary.
• Fisherfolk. The Lending Conduit shall coordinate with the Bureau of Fisheries and Aquatic Resources (BFAR) in the issuance of a
certification that the fisherfolk-borrowers under PLEA are registered under the Municipal Fisherfolk Registration (FishR). A copy of the
FishR certification shall be attached to the enrollment form and thus, verification of Part II by the Municipal Agricultural Office is no longer
necessary.
• ARBs & Coconut Farmers. If the borrower is an Agrarian Reform Beneficiary (ARB), the MAO shall seek the verification of the Municipal
Agrarian Reform Office (MARO). For borrowers whose main farming activity is coconut, the MAO may also seek the verification of the
Philippine Coconut Authority (PCA) through its designated official.

DEFINITION OF TERMS
1. Person with Disability (PWD): refers to "those suffering from restriction of different abilities, as a result of a mental, physical or sensory
impairment, to perform an activity in the manner or within the range considered normal for a human being." (R.A. 7277)

2. Household: refers to "a social unit consisting of a person living alone or a group of persons who sleep in the same housing unit and have a
common arrangement in the preparation and consumption of food." (PSA's Official Concepts and Definitions for Statistical Purposes)

3. Indigenous Cultural Communities (ICCs)/Indigenous Peoples (IPs): refer to "a group of people or homogenous societies identified by
self-ascription by other, who have continuously lived as organized community on communally bounded and defined territory, and who have,
under claims of ownership since time immemorial, occupied, possess customs, tradition and other distinctive cultural traits, or who have, through
resistance to political, social and cultural inroads of colonization, non-indigenous religions and culture, become historically differentiated from
the majority of Filipinos." (R.A. 8371)

4. Farm Parcel: refers to "a parcel of the holding that is one contiguous piece of land under one form of tenure without regard to land use. Both
the contiguity and one form of tenure conditions should be met for a piece of land to be classified as one parcel. Contiguous means that the
piece of land is not separated by more or less permanent natural or man-made boundaries like roads, rivers, and others that are not parts of the

5. Tenurial Status: refers to the "right of the farmer/ARB-farm laborer to use or operate the farm parcel." (RSBSA Data Items Concepts and
Definitions, NSO)

6. Registered Owner: refers to the occupant who "is the owner or co-owner who has legal possession of the farm/parcel or legal documentary
evidence/s to show ownership or right to own the farm/parcel." (RSBSA Data Items Concepts and Definitions, NSO)

7. Tenant: refers to "the person who, by himself/herself and/or with the aid available from within his immediate farm household, cultivates the
land belonging to, or possessed by another, with the latter's consent for purposes of production, sharing the produce with the landholder under
the share tenancy system, or paying to the landholder a certain amount of ascertainable in produce or in money or both, under the leasehold

8. Lessee: refers to "the person who, by himself/herself and/or with the aid available from within his/her immediate farm household, cultivates
