CONVERSION DISORDER

Summary
The client S. F was 27 years old married having 4 daughters housewife came to L.G.H with complaints
of Headache , jerky movements attacks of scizure , weakness and paralysis . The client was referred for
the assessment and management for her problems .The client was diagnose as having conversion
disorder .The conducted sessions in which differents techniques like deep breathing vocational training
suggestion psychoeducational were used with her for the achievement of short term goals .

The client came to L.G. H with complaints of headache Attacks of scizures weakness and paralysis .

History of present illness


The client problems started in 2018 one year ago , client has 4 daughters . She conceived no son his
husband and mother in law always querreling abusing and beating her for having no son .They blaming
her for daughters and having no son . The behavior of her husband was not good with her . The
environment of home always depressed. Her husband beat her for the saying of her mother and not
proper look after the client and daughter. Behavior of client s husband with daughter was scary and
rude. This insulting neglecting behavior of her husband and mother in law became the client mently ill.

She was unable to perform her daily routine work properly. She started to neglecting her daughters her
conditions remained like that clients parents noticed the condition of their daughter and they consult a
doctor for management of the clients problems.

Past Psychiatric History

The client had no suffered from any major psychiatric illness.

Family History

Client Husband
The client husband was 35 years old a motor mechanic .He had a careless attitude towards the client.

Client mother in law


Client mother in law was a 60 years old woman. She had a rigid personality. Her behavior with client
was also abusing and neglecting.

General Home Environment


General home atmosphere was experienced by client was conflicted abusing blaming ignoring. Clients
husband behavior with her always neglecting abusing and baeting . Her Mother in law also had critical
querreling with her . Client daughters which ages were 9 years 8 years 6 years and 4 years. They always
scary and neglected from their father. Client reported that these behavior of her Husband and Mother in
Law became her patient of conversion.

Personal History
T he client was born at home through a normal delivery. As she heared from her mother. She achieved
all his developmental milestone at proper age.

The client was a hard working and devoted person for her home. She proper look after her Husband
daughters and Mother in law.

Client behavior was observed through general discussion and unstructured interview.
1. Clinical Interview
A detailed Clinical interview was conducted with client.
2. Mental State Examination
Mental State Examination was done to assess the current level of cognitive function of the client
The client was average height and weight. She was dressed neat. She maintained poor eye
contact. Her speech was appropriate but seem confused . She became anxious while sharing her
history. Her perception attention and concentration was appropriate. Her recent and remote
memory was intact. She was fully oriented to time person place. No delusion Hallucinations
were reported by client. Depersonalization and Derealization were not found in client. Insight
was present.

Checklist of conversion Disorder
RISB was administered on client to assess her personality.
Score of RISB IS 157
Interpretation
Score of RISB shows that person has a maladjusted personality.
Provisional Diagnosis
Case Formulation
Conversion Disorder is characterized by with weakness and paralysis , abnormal movements
attacks of scizures cause anxiety and distortion in individual. In this case client experienced
these symptoms and she fulfill the criteria of conversion disorder according to DSM5.
Drug Treatment
The client was prescribed following medications by consultant Psychiatrist
Tricyclic antidepressant drugs Heloperidol .

Management Plan
The Management plan was made by as follows.
Short Term Goals
 Developing the therapeutic alliance with Client and motivating the client to indulge in
the therapy for treatment.
 Deep breathing was taught to client as a strategy for general stress reduction
Long Term Goals
 Continuation of short term goals in follow up.
 Change the old ways of feelings and thinking and free oneself from these
vulnerabilities.
 Maintaining a healthy work life balance.

Sessions
1st Session (35 minutes)
The therapist helped out the client by adopting the active listening and unconditioned
acceptance to her as a result of this she started sharing her history openly .The client was involved in the
therapeutic process . Presenting complaints and history of present illness was obtained through clinical
interview formal assessment was taken.

2nd Session (40 minutes)


Psychoeducation was provided to client around general structure of therapeutic process and the
concept of home work assignments as integral parts of therapy was given in this session. She was
educated about the number of sessions and duration of sessions and treatment procedures. Sportive
work was done with the client by providing her the empathy and unconditioned positive regard as a
result of this , she became aware of the procedure and schedule of the therapeutic session which
helped the client.
