It has come to my attention that you took 3-day leave from your job without requesting or
receiving prior permission from your manager. This kind of behavior is seen as a sign of gross
negligence of your duties that goes against our company's employee attendance policy.

You are hereby warned not to continue this inappropriate behavior in the future. If you do, you
shall invoke the appropriate disciplinary action. But since you have never taken leave without
permission before, I would like to request you to submit a written explanation for taking
unauthorized leave within three working days of receiving this letter.

A copy of this letter will be put in your office personnel file. Kindly treat this as an urgent
matter.

SUBJECT: Wearing of official uniforms/IDs

All employees/workers of Riso Paperu are expected to wear the prescribed uniform unless the
day's tasks require otherwise. The official identification cards must also be worn at all times.

Managers are expected to inform employees when they are violating the dress code. Employees
in violation are expected to immediately correct the issue. This may include having to leave work
to change clothes.

early for our employees. The said schedule may not make them productive and efficient. In
addition, there will be no available time for inspection and maintenance check-up activities. It
will be best to start the production at a later time than the original schedule in order to make the
necessary pre-production activities.

Thank you for your continuous efforts for the company. If you receive this message, please make
the revisions as soon as possible in order to effect the changes immediately.
