AFFIDAVIT OF DISCREPANCY
I, ROBERT LEONG WONG, of legal age, Filipino, married and a
resident of 0673 Aurora 6th St., Digos City, Davao del Sur, Philippines,
after having been duly sworn to in accordance with law, do hereby
depose and state:

That I am the same and identical person who voluntarily caused for
the execution of this Affidavit;

That in my Driver’s License, Professional Regulation Commision

Records, School Records, Marriage Certificate, PAG-IBIG Records and
PhilHealth Records, my middle name was spelled as LIONG;

That the true and correct spelling of my middle name is LEONG

and not LIONG as shown in my Passport and PSA Birth Certificate;

That the name ROBERT LEONG WONG and ROBERT LIONG

WONG refers to one and the same person;

That I am executing this Affidavit in order to the attest the truth of

the foregoing facts, to apprise the proper authorities of the same and for
whatever legal purpose this may serve best;
