AFFIDAVIT

I, AVITA G. MENDOZA, of legal age, married, Filipino and with residence at 178-A S. Jayme
St., Zone Gabi, Pakna-an, Mandaue City, after being sworn in accordance with law, hereby depose
and state, THAT:

1. My husband, Oscar A. Mendoza, and I have recently purchased a parcel of

land with improvements located in Brgy. Cabancalan, Mandaue City, Province
of Cebu, consisting of a total of Eighty One (81) square meters, more or less
and covered by TCT No. 35274. A copy of Deed of Absolute Sale is hereto
attached as Annex "A" ;

2. On the said parcel of land, an improvement was introduced by the former

owners thereof. The said building was duly declared for tax purposes and
covered by Tax Declaration No. 2017-009-02754. As appearing on the tax
declaration, the improvement's total assessed value is One Hundred Thirty-
Three Thousand Seven Hundred Twenty Pesos (P133,720.00);

3. On an ocular visit of the property, I discovered that the building constructed

on the property was already dilapidated and beyond repair. The current state
of building no longer justifies the assessed value stated in the tax declaration;

4. Accordingly, I would like to respectfully request for a reassessment of the

property for the proper and just appraisal of the same for tax purposes; and

5. I attest to the truth of the foregoing statements to the best of my own

personal knowledge and based on authentic documents or records. I am
executing this Affidavit to support my request for a reassessment of the
building covered by Tax Declaration NO. 2017-009-02754 with Property Index
No. 151-04-009-19-001-1001 as well as for all lawful intents and purposes it
may serve.
