Romiplostim in Patients With Immune Thrombocytopenia
Amgen recommends the use of its products only in accordance with the approved local product label.1
Clinical data evaluating the use of romiplostim in patients with immune thrombocytopenia (ITP)
include two pooled analyses of clinical studies,2-4 three phase 3 studies,5-7 a post hoc retrospective
analysis,8,9 a long-term extension study,10 and two large observational studies.11,12
A pooled analysis evaluated the safety and efficacy of romiplostim in splenectomized and
nonsplenectomized adults with primary ITP based on data from 13 clinical studies conducted

between 2002 and 2014. Patients received subcutaneous (SC) romiplostim, placebo, or standard
of care (SOC), along with concomitant or rescue medications (including intravenous
immunoglobulins but excluding other thrombopoietin [TPO] mimetics and investigational

products) as required and allowed per study protocol. Platelet counts were targeted to be in the

A platelet response (defined as ≥ 1 platelet count ≥ 50 x 109/L without rescue medication use in

In each treatment group, exposure-adjusted rates of adverse events (AEs), serious AEs (SAEs),
treatment-related AEs, and treatment-related SAEs were higher among splenectomized vs
nonsplenectomized patients. The rate ratio for exposure-adjusted AE rates (romiplostim vs
placebo/SOC) was 0.66 (1,226 vs 1,857 per 100 patient-years) among splenectomized patients
and 0.81 (852 vs 1,052 per 100 patient-years) among nonsplenectomized patients.
Exposure-adjusted rates of SAEs in the romiplostim vs placebo/SOC group were 49% lower in
splenectomized patients (68.1 vs 133.9 per 100 patient-years) and 53% lower in
nonsplenectomized patients (44.1 vs 94.2 per 100 patient-years). Exposure-adjusted rate of any
SAE was highest in splenectomized patients in the placebo/SOC group, followed

platelet count of 50 to 200 x 109/L.3 Data from the placebo/SOC arms were pooled. Patients
received romiplostim during the study for up to 283 weeks and placebo/SOC for up to 18 months.3
Of the 1,059 patients, 61% were female and 85% were White/Caucasian; the mean (standard
deviation [SD]) age of all patients was 52 (18.4) years, including 23 pediatric patients (aged 0 to
17 years) and 294 geriatric patients (aged ≥ 65 years). A total of 921 patients received
romiplostim; 65 received placebo/SOC; and 73 received placebo/SOC and subsequent
romiplostim. The mean (SD) baseline platelet count was 20.6 (16.5) x 109/L, and the mean (SD)
number of years since ITP diagnosis was 7.4 (9.3) years. The mean (SD) treatment duration was
76 (64) weeks, and the total exposure to romiplostim was 1,520 patient-years. The mean (SD)
romiplostim dose used most frequently was 4.7 (3.6) mcg/kg.3
The most frequently reported AEs (rates per 100 patient-years) in the romiplostim vs
placebo/SOC groups were headache (60.9 vs 58.2), contusion (47.8 vs 50.0), epistaxis

(35.2 vs 52.7), and nasopharyngitis (34.4 vs 33.6).3,4 The incidence of AEs during the study is

summarized in Table 1. Five fatal bleeding events were reported in the romiplostim group
(intracranial hemorrhage, n = 2; cerebral hemorrhage, n = 1; pulmonary hemorrhage, n = 1) and

(47.8), epistaxis (35.2), and petechiae (25.1).3,4 The most common thrombotic events (rate per

100 patient-years) in patients receiving romiplostim were DVT (1.1), PE (0.9), and myocardial
infarction (MI; 0.6). Romiplostim was discontinued in nine of the 15 patients with reticulin AEs,
and the dose was reduced in two patients. Neutralizing antibodies to romiplostim were detected
in three patients; however, they did not develop neutralizing antibodies to endogenous TPO.3 The

PHASE 3 STUDIES
PLACEBO-CONTROLLED STUDIES
Two parallel, double-blind, placebo-controlled, phase 3 studies evaluated the efficacy and safety
of romiplostim in patients with chronic ITP. One study was conducted in patients who had
undergone splenectomy (N = 63), and the other was conducted in patients who had not
undergone splenectomy (N = 62). Adult patients with a mean baseline platelet count ≤ 30 x 109/L
were randomized 2:1 to receive romiplostim or placebo. The starting dose of romiplostim was
1 mcg/kg SC QW, and the doses were adjusted to maintain a target platelet count of 50 to
200 x 109/L.5 Patients could continue stable doses of corticosteroids, azathioprine, or danazol
during the study or could be given rescue medications at any time at the discretion of the
investigator. Rescue medication was defined as a dose increase in concurrent ITP therapy or the

use of any new drug to increase platelet counts. After 24 weeks, the study treatment was

discontinued and patients were followed up to week 36 or until platelet counts were < 50 x 109/L.
The primary endpoint was the incidence of a durable platelet response, defined as a platelet count

The approximate median weekly romiplostim dose during the study was 2 mcg/kg in the

nonsplenectomized patients and 3 mcg/kg in the splenectomized patients. Safety data were
pooled for the romiplostim and placebo arms, with an incidence of any AE of 100% and 95%,
rso
respectively. Commonly reported AEs (≥ 20% of patients) in either treatment arm included
headache, fatigue, epistaxis, arthralgia, contusion, and petechiae. Severe, life-threatening, or
fatal bleeding events were reported for 12% of patients in the placebo arm and 7% in the
pe
romiplostim arm, invariably at platelet counts < 20 x 109/L. Treatment-related SAEs were reported
in two romiplostim-treated patients. One nonresponding patient developed increased bone
marrow (BM) reticulin after 7 weeks of therapy, with a return to baseline after discontinuing
so
romiplostim. Another patient, with extensive peripheral vascular disease, atrial fibrillation, and a
prior radial artery thromboembolectomy, developed a right popliteal artery thrombosis that was

successfully treated, allowing continuation on the study. Two patients in the placebo arm died
during the study, and one patient died 5 weeks after the study completion. One patient who
received romiplostim died of intracranial hemorrhage (after starting aspirin to treat thrombosis and

used to assess QOL in the ITP Patient Assessment Questionnaire compared with patients treated

(P = 0.02), Psychological scale (P = 0.049), Fear scale (P < 0.001), Overall Quality of Life scale
(P = 0.02), and Social Quality of Life scale (P = 0.002).7

In both arms, > 90% of the patients developed at least one AE, and the most common AEs
were headache and fatigue. One romiplostim-treated patient, in whom reticulin had not been
detected prior to treatment, was found to have grade 2 BM reticulin (within normal range) during
the 6-month post-treatment safety-monitoring period. Serious AEs occurred in 23% of patients
receiving romiplostim and 37% receiving SOC.7 The most common SAE was thrombocytopenia,
which occurred in 3% of the romiplostim-treated patients and 12% of the SOC-treated patients.
Three deaths occurred during the treatment period—one in the romiplostim group (pneumonia)
and two in the SOC group (hepatic failure and cardiorespiratory arrest).7 The AEs of interest are
summarized in Table 8.

A post hoc retrospective analysis evaluated the efficacy and safety of romiplostim in patients with
ITP ≥ 65 vs < 65 years of age using efficacy data from three randomized studies (two international

A total of 159 patients were included in the efficacy analysis (international phase 3 studies,
n = 125; Japanese study, n = 34), of which 39 (24.5%) were ≥ 65 years old and 120 (75.5%) were

< 65 years old. Of the 159 patients, 105 (66.0%) received romiplostim and 54 (34.0%) received
placebo/SOC. Mean baseline platelet counts for the subgroups ranged from around
16 to 18 x 109/L. Among patients ≥ 65 years old, 46.2% (18/39) had undergone splenectomy

compared with 50.0% (60/120) of patients < 65 years old; 92.3% (36/39) of patients ≥ 65 years
old completed the study compared with 94.2% (113/120) of patients < 65 years old. The overall

platelet response rates and mean number of weeks with a platelet response for romiplostim vs
placebo/SOC were slightly higher in patients ≥ 65 years old compared with those < 65 years old.8
Efficacy results by age are summarized in Table 9.

Of the 1,037 patients included in the safety analysis population, 295 (28.4%) were ≥ 65 years old;
65 (6.3%) patients received placebo/SOC only; 69 (6.7%) received placebo/SOC followed by
romiplostim; and 903 (87.1%) received only romiplostim. Baseline platelet counts ranged from
18.1 to 25.6 x 109/L for the various patient subgroups.8 Of patients ≥ 65 years old, 28.8% (85/295)
of patients had undergone splenectomy compared with 41.8% (310/742) of patients

< 65 years old. The median (quartile [Q]1, Q3) duration of exposure to romiplostim and median
average weekly romiplostim dose was 60.0 (28.0, 118.8) weeks and 3.0 (1.9, 5.5) mcg/kg for
patients ≥ 65 years old and 54.0 (25.7, 114.1) weeks and 3.7 (2.0, 6.1) mcg/kg, respectively, for
patients < 65 years old.8 Among patients ≥ 65 years old, 95.6% and 92.9% of those treated with
romiplostim and placebo/SOC, respectively, reported at least one AE; among patients < 65 years
old, 93.4% of patients in both romiplostim and placebo/SOC groups had at least one AE. Serious
AEs were reported for 40.4% and 38.1% of the romiplostim- and placebo/SOC-treated patients,
respectively, among patients ≥ 65 years old and for 29.4% and 28.6%, respectively, among
patients < 65 years old.9 Overall, the most frequently reported AEs were headache, contusion,
epistaxis, nasopharyngitis, and fatigue. The most frequently reported SAEs (duration-adjusted
rates) were thrombocytopenia, pneumonia, and gastrointestinal hemorrhage. Duration-adjusted
rates of arthralgia, nausea, peripheral edema, thrombocytopenia, hematoma, dizziness, myalgia,
and vomiting were slightly and consistently higher in the romiplostim-treated patients compared

Safety and efficacy were evaluated in an open-label, long-term extension study in patients with
ITP who had completed a previous romiplostim study. Patients previously randomized to
romiplostim received the same starting dose as their last dose in the previous study or 1 mcg/kg
SC QW if ≥ 24 weeks had elapsed since their last dose. In patients previously randomized to
placebo, romiplostim was started at 1 mcg/kg SC QW. The romiplostim dose was adjusted, as
needed, to achieve and maintain a platelet count between 50 and 200 x 109/L. Concurrent
ITP medications (corticosteroids, danazol, and azathioprine) were permitted at the same dose
and schedule administered prior to the initial romiplostim study.10 The efficacy endpoints of the
study were platelet response, defined as a platelet count ≥ 50 x 109/L and double the baseline
value (in the absence of rescue medication in the preceding 8 weeks), and the proportion of
patients using other ITP therapies and patients who required rescue treatment.10

At study completion, 291 adult patients were treated with romiplostim and 200 completed the
study. The median time since diagnosis in splenectomized patients was 9.77 years compared
with 3.24 years in nonsplenectomized patients. Of the 37 patients who were receiving concurrent
ITP medications at baseline, 54% (30/37) had discontinued concurrent ITP therapy. The mean
duration of romiplostim treatment was 110 weeks (range, 1 to 277 weeks). Ninety-five percent of
patients achieved a platelet count ≥ 50 x 109/L during the study. The median percentage of time
on study with a platelet response was 92% (range, 62% to 100%). Most patients (63%) responded
by week 2, after a single dose of romiplostim, and the rate of platelet response remained between
62% and 78% through week 212 (< 20 patients were on study after week 212).10
Ninety-eight percent of patients developed at least one AE, and 8% were considered serious,
treatment-related AEs. The most commonly reported AEs were headache, nasopharyngitis, and
fatigue. Thrombotic events occurred in 25 (9%) patients. Bone marrow biopsies were performed
.
at the investigator’s discretion; of the 40 biopsies performed, increased BM reticulin was reported

in 11 patients. Two patients developed neutralizing antibodies to romiplostim, which did not cross-
react with endogenous TPO and were absent following romiplostim discontinuation. Sixteen

deaths occurred during the study, with two deaths considered by the investigator as possibly
related to treatment (unstable angina and MI).10

A large, prospective, observational study conducted in Europe evaluated the use of romiplostim
in a clinical practice setting in 356 patients with ITP. Patients enrolled were ≥ 18 years of age who

In the FAS, the rate of grade ≥ 3 bleeding was 12.1 per 100 patient-years before romiplostim
administration and 2.2 per 100 patient-years after romiplostim initiation; the rate of ITP-related
hospitalizations was 87.2 per 100 patient-years before romiplostim administration and 33.3 per
100 patient-years after romiplostim initiation. Seventy-three (22%) patients experienced a total of
156 ADRs (32.3 per 100 patient-years).11 Thirteen (4%) patients experienced 18 serious ADRs
(3.7 per 100 patient-years), including three events of loss of response; two events each of DVT,
PE, and myelofibrosis; and one event each of thrombosed hemorrhoids, MI, phlebitis, retinal vein
thrombosis, transient ischemic attack, thrombocytosis, acute myeloid leukemia, chronic myeloid

leukemia and polycythemia. Two patients discontinued romiplostim after BM fibrosis. Adverse
drug reactions were reported, and follow-up biopsies were not available. No fatal ADRs were
reported.11
Another large, European, observational study (PLATON) reported the final outcomes of
romiplostim treatment in real-world clinical practice in 100 patients with ITP, according to platelet
response, bleeding and hospitalization rates, and safety.12 Patients enrolled were ≥ 18 years old
and had received romiplostim in Bulgaria, Czech Republic, Slovenia, Slovakia, Romania, and
Russia. In this study 104 patients were enrolled, 100 patients were analyzed, and 73 patients
(73%) completed the study. Four patients were excluded due to unmet eligibility criteria, reporting
error, or were lost to follow-up immediately following enrollment.12

Data from the pooled analyses of clinical trials2,3 showed that long-term romiplostim treatment
was well tolerated, with no new safety signals, even in patients treated for up to 5 years. Platelet

In two placebo-controlled, phase 3 studies, patients who received romiplostim achieved higher
incidences of durable platelet response (platelet count ≥ 50 x 109/L for at least 6 of the final
8 weeks of treatment period in the absence of rescue medications at any time during the 24-week
treatment period) compared with patients treated with placebo. Rescue therapies were required
by 21.7% and 59.5% of patients receiving romiplostim and placebo, respectively. The most
common AEs across both arms were headache, fatigue, epistaxis, arthralgia, contusion, and
petechiae.5,6 In the other phase 3 study, patients treated with romiplostim had fewer treatment
failures (11% vs 30%; P < 0.001), had fewer splenectomies (9% vs 36%; P < 0.001), and were

SOC. The most common AEs across both treatment arms were headache and fatigue.7

In a post hoc, retrospective analysis, the overall platelet response rates and mean number of
weeks with a platelet response for romiplostim vs placebo/SOC were slightly higher in patients
≥ 65 years old compared with those < 65 years old. Duration-adjusted rates of arthralgia, nausea,
peripheral edema, thrombocytopenia, hematoma, dizziness, myalgia, and vomiting were slightly
and consistently higher in the romiplostim-treated patients compared with placebo/SOC-treated
patients among both age groups.8

Use of Romiplostim in Pediatric Patients
With Chronic Immune Thrombocytopenia

Clinical data evaluating the use of romiplostim in pediatric patients with immune thrombocytopenia
(ITP) are available from a pooled analysis of clinical studies2-4, a randomized, double-blind phase
3 study,5 a phase 1/2 study,6,7 two phase 3 extension studies,8-10 and a single-arm phase 3
study.11,12

In an integrated analysis of 5 clinical trials, the safety and efficacy of romiplostim were evaluated
in 286 pediatric patients with ITP; data were pooled from two placebo-controlled (one phase 3
and one phase 1/2) trials, two open-label (phase 3) extension trials, and a single-arm (phase 3)

Incidences of any bleeding and noncutaneous bleeding during the course of the study (including
time before an effective dose was reached) were similar in both groups. In a post hoc analysis,
duration-adjusted rates (per 100 patient-weeks) of all bleeding (30.9 in the romiplostim group vs
47.3 in the placebo group; P < 0.0001) and CTCAE grade 2 or higher bleeding (5.9 in the
romiplostim group vs 17.9 in the placebo group; P = 0.0006) were both significantly lower with
romiplostim than with placebo.5
Overall, no deaths or patient withdrawals occurred due to adverse events (AEs), no cases of
thrombotic or thromboembolic events, and no neutralizing antibodies to either romiplostim or
thrombopoietin (assayed at baseline, week 12, and week 25) were reported. No new safety
signals were identified in this study with respect to the known safety profile of romiplostim.5 Safety
results are summarized in Table 5.

A phase 1/2, randomized, double-blind, placebo-controlled study evaluated the safety and
efficacy of romiplostim in pediatric patients with ITP. Pediatric patients with ITP ≥ 6 months

Patients who completed the phase 1/2 study, the first open-label extension study, or the

term phase 3 extension study.8-10 Sixty-six patients were enrolled; one patient withdrew before

age was 11 (range 3 to 18) years; median baseline platelet count was 28 (range 2 to 458) x 109/L.

During months 0 to 6, the median (Q1, Q3) percentage of time with a platelet response was 50%
(17%, 83%), which increased to 83% (50%, 100%) during months 7 to 12. The median (Q1, Q3)
percentage of time with an increase in platelet counts ≥ 20 x 109/L above baseline was 74% (39%,
90%). Four patients experienced remission with platelet counts ≥ 50 x 109/L and no ITP
medication for ≥ 24 weeks. In a multivariate analysis, patients reaching a dose of 10 mcg/kg were
more likely to have a baseline platelet count < 10 x 109/L (P < 0.0001), prior splenectomy (P =
0.005), and age < 6 years (P = 0.02).11,12
Of 203 patients who received romiplostim, 64 discontinued treatment (lack of efficacy, n = 38;
AEs, n = 7). The most frequent AEs reported during the study were epistaxis (32%), headache
(31%), and viral upper respiratory tract infection (28%). Five patients experienced SAEs that were

deemed treatment related by the investigator (headache, n = 2; abdominal pain, n = 2;

presyncope, n = 1; and neutralizing antibodies, n = 1). Bleeding events were reported in 62% of

SUMMARY
Clinical data on the use of romiplostim in pediatric patients with ITP are available from a pooled
analysis of 5 clinical studies,2,3 a phase 3 study,5 a phase 1/2 study,6,7 two open-label phase 3
studies,8-10 and a single-arm phase 3 study.11,12 Across all clinical studies, the platelet response
was reported in 71%–94% of patients, durable platelet response ranged from 52%–56%, median
range of average weekly romiplostim dose was 3.9–6.9 mcg/kg, and median treatment duration
ranged from 7–124.8 weeks.2,3,5-12 No new safety signals were identified with respect to the known

safety profile of romiplostim. Neutralizing antibodies to romiplostim were reportedly low and no
neutralizing antibodies developed to endogenous TPO.2,3,5-12
In the pooled analysis of these 5 clinical trials, 286 children received romiplostim for a total
duration of 468 patient-years. Most (89%) patients had a platelet response with 19 of those
patients achieving remission. In those who achieved remission, platelet counts were maintained
for a median of 12 months. The average median weekly dose of romiplostim was 6.6 mcg/kg and
the median duration of romiplostim treatment was 65 weeks. Treatment-related SAEs were
observed in 24% (17% with persistent ITP and 27% with chronic ITP) of patients. No fatal AEs
were reported. Seven patients reported neutralizing antibodies against romiplostim: 2 transient
and 5 persistent; no neutralizing antibodies to endogenous TPO were reported.2,3,3
.
In the phase 3, double-blind, placebo-controlled study, 71% of patients reported a platelet

response and 52% reported a durable platelet response. Durable platelet response rates with
romiplostim by age group were 38% (3/8) for 1 to < 6 years, 56% (10/18) for 6 to < 12 years, and

In the randomized phase 1/2, placebo-controlled study of pediatric patients with ITP of ≥ 6 months’
duration, platelet counts remained ≥ 50 x 109/L or increased by ≥ 20 x 109/L from baseline for at

El criterio de evaluación primario fue la incidencia de respuesta duradera, definida como el alcance del
recuento plaquetario de ≥ 50 x 109/L durante al menos 6 semanas de la semana 18 hasta la semana
25 de tratamiento. En general, una mayor proporción significativa de sujetos en el grupo de romiplostim

Los errores en la dosificación, incluyendo sobredosis e infradosis, se han notificado en pacientes que
reciben Nplate, se deben seguir las guías de cálculo de dosis y ajuste de dosis. En algunos pacientes
pediátricos, la dosificación exacta depende de una dilución adicional tras la reconstitución, lo que
puede aumentar el riesgo de errores de medicación (ver sección dosis y vía de administración).
La sobredosis puede dar lugar a un aumento excesivo en los recuentos plaquetarios y asociarse con
complicaciones trombóticas/tromboembólicas. Si los recuentos plaquetarios aumentan excesivamente,
interrumpir Nplate y monitorear los recuentos plaquetarios. Reiniciar el tratamiento con Nplate de
acuerdo con las recomendaciones de dosificación y administración. Una dosis insuficiente puede dar
lugar a recuentos plaquetarios menores de lo esperado y posible sangrado. Los recuentos plaquetarios

Los estudios realizados en animales han mostrado que romiplostim atraviesa la barrera placentaria y
aumenta los recuentos plaquetarios en el feto. En estudios con animales, también ocurrió pérrdida
post-implantación y un ligero aumento en la mortalidad perinatal de las crías (ver sección precauciones

Las reacciones adversas se derivan de cada uno de los grupos aleatorizados de seguridad en PTI
pediátrica (2 estudios clínicos controlados) y de los grupos de seguridad en PTI pediátrica (2 estudios
clínicos controlados y 3 no controlados) donde la incidencia en los sujetos fue de al menos un 5%
superior en el grupo de romiplostim comparado con el grupo placebo y al menos un 5% de incidencia

Las reacciones adversas más comunes en pacientes pediátricos con PTI a partir de 1 año fueron
infecciones del tracto respiratorio superior, rinitis, tos, dolor orofaríngeo, dolor en la zona superior del
abdomen, diarrea, erupción, fiebre, contusión (notificados como muy frecuentes (≥ 1/10)), y faringitis,
conjuntivitis, infección en el oído, gastroenteritis, sinusitis, púrpura, urticaria y edema periférico
(notificados como frecuentes (≥ 1/100 a < 1/10)).
En los estudios pediátricos, se observaron las reacciones adversas adicionales: dolor orofaríngeo,
dolor en la zona superior del abdomen, rinitis, faringitis, conjuntivitis, infección en el oído, sinusitis y
edema periférico comparado con los observados en los estudios en adultos.
Algunas de las reacciones adversas observadas en adultos y que se notificaron en sujetos pediátricos

blásticas en pacientes tratados con romiplostim comparado con placebo. De los casos de progresión
de la enfermedad de SMD a LAM que se observaron, los pacientes con SMD con clasificación de riesgo
RAEB-1 al inico del estudio, fueron más propensos a presentar progresión de la enfermedad a LAM

Desde un punto de vista microbiológico, el producto se debe usar de forma inmediata. Si no se usa de

del usuario y no deberían superar las 24 horas a 25C ó 24 horas en refrigeración (entre 2C y 8C),

Tras la dilución: se ha demostrado estabilidad química y física durante 4 horas a 25C cuando el
producto diluido se mantuvo en una jeringa desechable, o 4 horas en refrigeración (entre 2C y 8C),
cuando el producto diluido se mantenía en el frasco ámpula original.
Desde un punto de vista microbiológico, el medicamento diluido se debe usar de forma inmediata. Si
no se usa de forma inmediata, los tiempos y condiciones de conservación antes de su utilización son
responsabilidad del usuario y no deberían superar las 4 horas a 25C en las jeringas desechables o
4 horas en los frascos ámpula originales en refrigeración (entre 2C y 8C), protegido de la luz.
