Dodgeball Tourney

Where:​ ​Strafford Max Evans Gymnasium. 
Time:​ ​9:00 a.m. 
Cost:​ ​$100/team; 10 players/team. 
*​Make checks out to “Strafford Youth Football Organization.”

TEAM DETAILS:
Each team ​MUST HAVE​ at least 2 girls. 10 players per team.

Deadline to Register:​ ​Friday, February 14.

Mail/drop off registration forms to the Strafford Middle School office. ​All money
raised will benefit the non-profit Strafford Youth Football and Cheerleading
Organization.

 Liability Waiver, Release and Consent
By signing this waiver, I agree the player has permission to attend and participate in the Strafford
Youth Football and Cheerleading Dodgeball Tournament. All players and parents/guardians (if a
player is under 18 years old) must sign this waiver in order to be eligible to participate in the
event. This waiver must be submitted ​before​ ​the first game.

Each of the undersigned registrants release, discharge, and agree to hold harmless the Strafford
Youth Football and Cheer board members, officers, sponsors, volunteers, and officials and the
Strafford R-VI School District and its employees from any and all claims, expenses, liabilities, or
causes of action for any injury or loss sustained by any player while playing, practicing,
traveling, or participating in the Strafford Youth Football and Cheerleading Dodgeball
Tournament. I understand and acknowledge this waiver is intended to be a full and final waiver
and release of any claim for injury or loss arising out of the player’s participation in the event.

If I, or my child is injured and requires medical care, I consent to have an athletic trainer, coach,
physician, dentist or associated personnel provide the player with medical assistance or treatment
and agree to be financially responsible for the cost of such assistance or treatment.
