JOINT AFFIDAVIT OF TWO

DISINTERESTED PERSON

WE, EDUARDO G. LARANJO and RIGOR M. MOTIO, both of

legal ages, Filipino citizen, and with residence address at Trece Martires
City, Cavite, after having been sworn in accordance with law hereby
depose and state:

1. That we personally known MELINDA QUARRE CARPIO the

biological daughter of SPS. ROGELIO O. CARPIO and ENORETA M.
QUARRE who was born on March 18, 1970 at Davao City, Davao
Del Sur;

2. That we are the friends and neighbors of MELINDA QUARRE

CARPIO;

3. That we are not related to by consanguinity and affinity;

4. That we are attesting the facts that MELINDA QUARRE CARPIO

we known her since birth;

5. That the Certificate of Live Birth of MELINDA QUARRE CARPIO

was not duly registered for the reason that the “MANGHIHILOT”
did not forward the Certificate of Live Birth of the said child;

6. That as consequence thereof they wish to registered from the

Local Civil Registrar Office of Davao City, Davao Del Sur, as Late
Registration;

7. That this affidavit is executed to attest to the foregoing and for

whatever legal purpose may serve.
