PERSONNEL TRANSFER AT SEA

INSTRUCTIONS

1. Mark the appropriate columns – if in doubt, do not hesitate to ASK the Master or OIM
2. All the checklist items must be answered YES or N/A (Not applicable). If any NO, the transfer must not
be conducted until that item is rectified.
3. For vessels whose duty is to transport people from one platform to another, provided the weather and
condition of vessel has not deteriorated, the check list may be completed only once a day i.e. for the first
transfer only. However, for transfer where the checklist is not filled up, the Master must exercise mental
assessment.

4. The checklist must be completed in cases where the transferee refused to be transferred. This is to
support the case.
5. The checklist must be completed in cases where the Master decides not to conduct transfer operations.
6. This checklist must be shown to all involved personnel before the transfer.

2. Are the boat/s motion relatively stable i.e. not pitching, heaving or rolling
heavily?
3. Is the sea state LESS than 2.5 metre?

4. Do all the transferee and assisting personnel have PPE on?

5. Are there personnel on the boat and / or at the platform boat landing to assist?

6. Has the boat Master and OIM approved the transfer?

7. If crew change, has LMR approved and OIM notified of the transfer?

8. Has the individual AGREED to the transfer in the prevailing conditions?

9. Are knotted swing ropes fitted and in good condition at the transfer points?

10. Is the transfer conducted in daylight hours, if not, is the area well illuminated and
visible?
11. Are both hands of the transferee free and not carrying any baggage?
12. Are all persons engaged in the work fully aware of Man Overboard Procedures?

13. Is there a Ring Life Buoy with a buoyant line available in the transfer area?

AGREED to the transfer as per this checklist?

OIM.

Semua perkara yang tersenarai didalam checklist ini mestilah dijawab YA atau T/B
(Tidak berkenaan). Kalau jawaban TIDAK, maka pemindahan tidak boleh dilakukan
sehingga perkara tersebut didapati sesuai dan selamat.

Bagi kapal-kapal yang ditugaskan untuk membawa kakitangan dari pelantar ke pelantar
dengan kerap kali dalam sehari, senarai ini dapat digunakan dengan sekali saja, iaitu bagi
pemindahan pertama. Ini tertakluk kepada keadaan cuaca dan kapal tidak bertambah
buruk dari waktu pemindahan pertama dijalankan. Pada pemindahan selanjutnya, di
mana pemindahan dijalankan tanpa memenuhi senarai, Nakhoda diingatkan agar
menjalankan senarai mental mengikut senerai ini.

bagi menunjukkan bukti mengapa pemindahan dibatalkan.

pemindahan?
2. Are the boat/s motion relatively stable i.e. not pitching,heaving or rolling heavily?
Adakah pergerakan kapal stabil? I.e tidak jongkang-jongkit, menghayun dan
goyang dengan kuat.
3. Is the sea state LESS than 2.5 metre ?
Adakah ombak laut kurang dari 2.5 meter?
4. Do all the transferee and assisting personnel have PPE on ?
Adakah semua orang yang berpindah dan mereka yang membantu semasa
pemindahan memakai Alat Perlindungan Keselamatan?
5. Are there personnel on the boat and / or at the platform boat landing to assist ?
Adakah terdapat anggota yang berada diatas kapal dan juga diatas pelantar
untuk membantu pemindahan?
6. Has the boat Master and OIM approved the transfer ?
Adakah nakhoda kapal dan OIM membenarkan pemindahan di lakukan?
7. If crew change, has LMR approved and OIM notified of the transfer ?
Kalau pertukaran anak kapal, adakah LMR membenarkan dan memberitahukan
Kepada OIM mengenai pemindahan tersebut?

Adakah semua setuju melakukan pemindahan setelah mengambil kira keadaan
cuaca pada waktu itu?
