With reference to the findings related to your questionable activities related to the issue of
“1.0 Offenses Against Conduct and Decorum / Order and Solidarity, 1.1, Gross and habitual
neglect by the employee of his duty” this is to notify you that as a result of careful
evaluation of the situation and presented documents, it has been found out that you have
committed lapses which are punishable under the Personnel Manual.

Following are the proven lapses:

1. Your performance as Branch Accountant catches attention because of your poor


performance as stated below:

A. First, you failed to print the Financial Statement dated April 30, 2017, the said FS is
very important as basis for the report of the branch that is submitted monthly in the Head
Office.
B. Second, you have two (2) months backlog in filing of the transactions of the Branch.
The filing of transaction is very crucial because the file that we have is our daily basis in our
transaction and we cannot afford of missing or losing any file.
C. Third, failure to give forms or requirements to the Unit regarding their request.
D. Lastly, you are caught playing games in your cellphone while on duties.
You have been negligent in the execution of your duties and responsibilities. Such actions
brought serious damage and risk to the Institution.

Being a Bookkeeper, it is assumed that you will be careful and cautious in your actions in
conformity with the policy of the Institution at all times. With the action above, it can be
concluded that the interest of the Institution was taken for granted because of your action
and decisions made which created damage and risk to the same.

Therefore, following the provision of the Personnel Manual and the decision of the
Management, it has been decided that you be DEMOTED from your current position as
BOOKKEEPER and assume the position of Administrative Assistant effective 1 November
2017.

It is expected that you will be more diligent and careful in the execution of your duties and
responsibilities and that you will cautiously follow the Provisions of the Personnel Manual.
Please be reminded that any violations or offenses that you may incur as stated in the
Personnel Policies can be grounds for receiving appropriate administrative sanctions
including losing employment with the company
